BEFORE YOU ARRIVE: Personal History Questionnaire procedures have changed at Diablo Canyon. All employees must complete an electronic PHQ prior to their first day of employment. Please select the PHQ link/description that corresponds to the last time you held an unrestricted keycard (UA) at an operating nuclear power plant. IMPORTANT-do not complete this more than two weeks prior to your hire date. Initial PHQ Initial Authorization; Persons in the process of gaining UA at a nuclear power plant for the first time, or after a lapsed clearance greater than 3 years. Complete this form 2 - 3 weeks prior to arrival to DCPP. (62 kbytes) Updated PHQ Updated Authorization; Authorization that will be restored during a period greater than 365 days but less than 3 years. Complete this form 2 – 3 weeks prior to arrival to DCPP. (51 kbytes) Reinstatement R1Y: An authorization that has been reestablished within 31-365 days since last favorable UAA/UA termination. This PHQ cannot be completed if you are still holding access to an operating nuclear plant. This form can only be completed after your last favorable termination of UAA/UA prior to DCPP and no earlier than 2 – 3 weeks prior to DCPP arrival. (51 kbytes) Reinstatement R30: An unescorted access authorization that has been reestablished within 30 days of last favorable UAA/UA termination. Do not complete this EPHQ if you are currently holding UAA/UA to another operating nuclear power plant. This form must be completed after your last favorable termination of UAA/UA prior to DCPP and no earlier than 2 weeks prior to DCPP arrival. (51 kbytes) FIRST DAY OF HIRE –In Processing with Fluor On 9.2.14, we will begin hiring at the Energy Education Center located at 6588 Ontario Road, San Luis Obispo (refer to Attachment 1). Unless notified, we will begin at 6am and the facility will be open at 5:45am. Please bring the following items: Work authorization documents, which include: Valid & current picture ID – Can include a driver’s license, State issued ID card, passport, military ID Social Security Card If not a US citizen – bring documentation Union dispatch Lunch/Snacks – there is no food or drink at this site SECOND DAY OF HIRE – In Processing with PGE On your second day, you will park in Lot 1 (refer to Attachment 2) and walk to Building 262-upstairs. This is located in Lot 7. You are expected to wear “work” clothes including long pants and work boots (steel toe is not necessary). Please clock in with the number provided to you on Day #1. The time clocks are located outside Building 262 - lower level. Please bring the following items with you: Picture ID that is valid and current Access Authorization (provided at Day 1 In Processing) Vehicle license plate numbers, vehicle make, model, year and color (form provided at Day 1 In Processing) Lunch (vending machines with limited snacks and drinks are available) If Applicable: Arrests in last 5 years – Bring court papers & documents with you HELPFUL INFORMATION The PGE section of the Diablo Microsite includes links to the most common qualification tests that you will be expected to complete. If you were a new or re-hire to Fluor or J. Givoo for the 1R18 outage in February 2014, you may only have a few brief Web Based Training classes to take because your training qualifies you for a one-year period. At this time, we are aware that the plant has added a Heat Stress and Scaffold test.
