A chaperone is available for both male & female
patients if required.
Administrative Staff
Surgeries are by appointment except for ‘Open
Surgeries’ and you can make your appointment in
person, by telephone or on-line. ‘Open Surgeries’
may change at short notice so please ring to confirm
the date, time and location of the next ‘Open
Surgery’. The Doctor will deal with ONE NEW
PROBLEM ONLY at an OPEN SURGERY.
It would be helpful if you could give the reason why
you wish to see the Doctor when you book an
appointment or attend an OPEN SURGERY so that
you can be directed to the most appropriate person.
You have the right to refuse this information.
You may request an appointment in the Nurse-led
Minor Illness Clinic, Monday-Friday, 10.30-11.30am
and 2.30-3.30pm. Please telephone the Practice to
confirm the location of the next Minor Illness Clinic. A
separate Minor Illness Information Sheet is available
either on-line or from Reception.

(Higher Folds & Leigh Health Centre both close at 6.30pm)
Saturday 8am to 12 Noon at our Bridgewater &
Wigan Road Sites.



Please note home visits are for patients who
are housebound.
Please contact the surgery before 10.30am if
on-the-day visit is required; giving the
patient’s name, address, telephone number
and symptoms.
The Doctor will decide whether a home visit
is justified and may telephone the patient to
help make this decision.
Please Note:
The Doctors and the Nurses are happy to speak to you on
the phone where appropriate if you need advice.
Leigh Family Practice is an accredited training practice with
dedicated GP trainers. We therefore regularly have one or
more ‘GP in training’ (ST2) attached to the practice. ST2s are
fully qualified doctors who are undertaking additional training
to qualify as GPs.
In addition, we regularly supervise training of medical
students from the local medical school. Please inform our
receptionists or your doctor if you would rather not have the
student present during your consultation.
If you have a URGENT problem you can be
seen on the same day.
Disabled Access
There are ramps at both Higher Folds and
Wigan Road Surgeries (the entrances at BMC
and LHC are flat), a lift at BMC and LHC and
disabled toilets at all four surgeries.

Home Visits

If you wish to register as a patient you should
ask the receptionists and they will give you a
registration form and a health questionnaire
form to fill in – it is important that you fill this
out in full. Alternatively, you can register online.
If you are on any repeat medications, please
bring proof of these so that we may add them
to the system (Re-order form from
prescription or boxes with printed pharmacy
labels).

Repeat Prescriptions




You can request a repeat prescription
either in person at the surgery, by post,
or on-line.
Please arrange with a
pharmacy of your choice if you wish your
prescription to be collected before you
submit your request.
Please allow two complete working days
before collection. Medication must be on
the repeat list.
Medication issued as an acute item will
not be re-issued until the patient has
seen the Doctor.
You may be asked to see the Doctor for
a medication review after 6 months
supply of medication has been issued.
Test Results
Please telephone the surgery between 10am
and 6pm to enquire about the results of your
test.

Rights & Responsibilities of Patients
* Patients can expect to be given the medical care
that they require and be treated in a polite and
efficient manner.
* Patients are entitled to see any GP.
* Staff have access to patient information. However,
this will not be divulged to other individuals including
relatives without the consent of the patient.
* We expect our patients to keep all their
appointments, arrive promptly & treat all staff politely.
* It is important for patients to attend for all reviews
when they are due, especially those with chronic
diseases.
