If you’ve already completed a SchoolChoice application online via Parent Portal: https://myportal.dpsk12.org , you DO NOT need to
complete a paper form.
TIPS ON COMPLETING THE SCHOOLCHOICE PROCESS SUCCESSFULLY

Take your time – There is no advantage to turning in a first round SchoolChoice Form early.

Visit your top choice schools – Contact the schools directly to find out about tours, open houses, and other opportunities to get to
know that school.

Complete the SchoolChoice Form and gather all required documentation – Use the SchoolChoice Form Checklist below as a
reference. If completing online, request a Parent Portal account no later than January 9, 2015.
4a. Be sure to complete section 4 of the SchoolChoice Form to receive any additional considerations that a school may offer and
you may be eligible for.

items listed on this checklist are acceptable forms of documentation. Incomplete SchoolChoice Forms cannot be processed.

I understand that my child is guaranteed enrollment at our neighborhood school if I have not “choiced out” of that school. I
also understand that once I have “choiced out” of my neighborhood school, I must follow the SchoolChoice process to return
to that school if I change my mind.
