Putting young people at the centre of planning
their lives and support

Background
The Children and Families Act requires everyone working with disabled children
and young people and those with SEN to support them to participate as fully as
possible in making decisions about their lives and support and local services.
This means taking account of the views, wishes and feelings of children and
young people.
But how do we really,

Have different conversations with young people and their families that
help them have good futures and
•
Ensure that we use this learning when developing education, health and
care plans and supporting people to transfer from statements and LDAs
Person centred tools and approaches have a key role to play in ensuring that
the voice of young people is placed at the centre of planning their lives and
support.
The Preparing for Adulthood team has been working with DfE, national
pathfinder champions and Helen Sanderson Associates to develop a new
resource on how to use person centred practice to develop education, health
and care plans for young people who are preparing for adulthood.
This workshop is to share the new resource and provide useful ideas on
how to;
•
•
•
•
have good conversations with young people about their aspirations
use person centred practice to develop good education, health and care
plans and transfer statements and LDAs to EHC plans
deliver preparing for adulthood reviews from Year 9 as described in the
SEND Code of Practice
achieve quality planning at scale and develop the workforce
•
develop a strategic approach to person centred practice and preparing
for adulthood pathways
•
To share examples of how good partnership working can support the
development of new provision; Traineeships, Apprenticeships, Supported
Internships
The opportunity to action plan and share learning with other providers and
LAs teams
Who should attend
This event is for all practitioners who are responsible for helping young people
aged 14-25 to develop education health and care plans.
It may therefore be useful for,
