The passport holder must complete the letter (including name and address of the representative), and the passport
holder’s signature which is the same as the signature in his/her passport.
申請者が全ての欄（代理人の氏名及び住所を含む）を記入し、パスポートと同じ署名をしてください。
※Representatives are required to bring their official PHOTO ID in original and copy, applicant’s passport copy to
certify authorization sign, and the original receipt, failing which the processed application will NOT be handed over.
