下記の事項を読んで、署名をしてください。Please read the following items and sign at the bottom.

私はこの申込書を提出し申請料を支払うことにより、カイ日本語スクールに留学の在留資格認定証明書交付申請の手
続きを依頼いたします。
By submitting this application and paying the application fee, I request KAI Japanese Language School to apply for a certificate
of eligibility on my behalf. A certificate of eligibility is required for students who wish to obtain a college student visa.

カイ日本語スクールが書類審査によって申請が不可能と判断した場合を除き、私はどのような理由でも申請料の返金
は請求いたしません。
Once my application documents and application fee have been submitted to KAI Japanese Language School, the school will first
review the application. If the application is judged to be sufficient, the school will process and submit the application to the
immigration authorities on my behalf. In this case, my application fee cannot be refunded. If my documents are judged by KAI
Japanese Language School to be insufficient, my application fee will be refunded.

私はカイ日本語スクールでは日本語以外の授業（英語・数学等）がないことを承知いたします。
I understand that KAI is a Japanese language school. KAI Japanese language school teaches only the Japanese language. (There
are no other subject like English, Math lessons, etc.)

入学後は、私はカイ日本語スクールの規則に従い、学生として責任を持って行動します。
Upon beginning my studies at KAI Japanese Language School, I agree to abide by the school rules and by its teachers’
instructions. In addition, I will, of my own initiative, recognize and act on my responsibility as a student.

私の在学中に私、私の両親、経費支弁者、日本の親族の連絡先が変わった場合、速やかに学校に連絡します。
If I, my parents, sponsor or relatives living in Tokyo change their address while I am studying at Kai Japanese Language School, I
will immediately report any such changes to the school.

下記の返金規定（留学ビザ申請の場合）について理解し同意いたします。
I understand well and agree to following refund policy (for college student visa applicants)
返金規定(留学ビザ申請の場合) Refund policy (for College Student Visa Applicants) ● ビザ申請書類提出後のキャンセルの場合；ビザ申請手数料は返金しません。
Cancellation of course after visa application documents have been submitted to KAI ; The visa application fee will be retained by the school
● 入国管理局から「在留資格認定証明書」交付後で授業料支払い前のキャンセルの場合；ビザ申請手数料は返金しません。
Cancellation of course after a “Certificate of Eligibility” has been issued by the Immigration Authority of Tokyo, but before school fees have been
paid ; The visa application fee will be retained by the school.
● 入国管理局から「在留資格認定証明書」交付後で授業料支払い後、且つ授業開始前のキャンセルの場合；理由の如何に関わらず入学
金は返金しません。またキャンセル料として１学期分の授業料を申し受けます。差し引きした授業料は在留資格認定証明書と入学許
可証と引き換えの上、返金いたします。
Cancellation of course after a “Certificate of Eligibility” has been issued by the Immigration Authority of Tokyo, after school fees have been paid,
and before commencement date ; Registration fee and Cancellation Fee equivalent to 1 term tuition fee will be retained by the school under all
circumstances. Balance of tuition fees will be refunded upon receipt of the original “Certificate of Eligibility” and certificate of admission.
● 日本大使館、領事館によってビザ発給が拒否された場合；入学金は返金しません。授業料はビザ発給拒否が当校職員によって確認さ
れた場合、全額を返金いたします。
Cancellation in the case of the applicant’s visa being rejected by the Japanese Embassy or Consulate ; Registration fee will be retained by the
school. Tuition fees will be refunded in full upon presentation of a letter from the Japanese Embassy or Consulate proving that the visa
application has been rejected.
● 授業開始後のキャンセルの場合；理由の如何に関わらず入学金は返金しません。またキャンセル料として１学期分の授業料を申し受
けます。当校に６ヶ月以上在学した場合、文書での届出により、提出時点で開始していない学期の授業料について返金いたします。
Cancellation of course after commencement date ; Registration fee and Cancellation Fee equivalent to 1 term tuition fee will be retained by the
school under all circumstances. If you withdraw from a course having studied at KAI for longer than 6 months, upon notifying our office in
writing, we will refund the tuition fees for any remaining terms that have yet to begin.
● 入学を延期した場合、既に支払われた授業料はその学生本人の授業に対してのみ振り替える事が可能です。
In the case of postponement, tuition fees are transferable for the original applicant.
この入学申込書に記入した内容が事実であることと上記の 7 項目に関し、同意したことを私の署名によっ
て誓約します。記載内容に変更が生じた場合は直ちに報告します。
My signature implies that all information submitted on my application is true. And I agree to the 7 items listed above.
If there is a change of status once this form has been filed, please inform Kai Japanese Language School immediately.
