Clarify the responsibility • Intentional data manipulation or missing data, primary, organization shall be responsible to the incidence • Such situation could not identify during verification, VV shall also be part of the responsibility. Countermeasure －Spot check, suspension, sanction • AB shall conduct action plan related with the incidence timely, if necessary, AB shall decide what and when to review at spot check. According to the review, AB shall ask committee to consider the final decision in terms of suspension, if necessary.
