Starting and titrating V-Go Insulin Delivery Device ® V-Go Actual Size A prescriber’s guide to dosing Start Choose the appropriate V-Go® option based on the patient's body weight. • V-Go delivers a continuous subcutaneous insulin infusion (CSII) • CSII typically reduces total daily insulin dose by 25% to 30%1 Adults on basal insulin only (± OADs)2,3 Patient’s body weight ≤220 lb >220 lb x 0.45 kg/lb x 0.5 U/kg Basal rate V-Go 20 Units/24 hours Basal rate V-Go 30 Units/24 hours Bolus dosing 2 clicks/meal Bolus dosing 3 clicks/meal 1 click = 2 Units 1 click = 2 Units Adults on MDI or premixed insulin1 Patient’s body weight (kg) x 0.5 50% basal rate Basal rate V-Go 20, 30, or 40 Units/24 hours OAD = oral antidiabetic drug 50% bolus dosing + Divide bolus doses between meals Maximum V-Go bolus capacity: 36 Units/day in 2-Unit increments Titrate Optimize V-Go bolus dosing before adjusting the basal rate. 1. Titrate mealtime clicks each week until patient reaches preprandial goal 2. If fasting glucose is still not optimized, consider switching to the next V-Go option3 Premeal blood glucose consistently not at goal* Preprandial glucose 70-130 mg/dL†4 1. Add 1 click to previous meal‡ 1 click = 2 Units Titrate bolus dosing weekly until optimized Premeal goal achieved If fasting blood glucose is also at goal, continue to assess weekly Premeal goal not achieved All clicks used 2. If fasting blood glucose is not at goal Consider increasing basal rate to next V-Go option Decrease bolus dose by 1 to 2 clicks per meal Fasting glucose >180 mg/dL†5 *If the median of the weekly premeal readings is above target, continue to titrate the bolus dose. Customize for each patient. † For example, if a predinner reading is consistently high, adjust the bolus dose at lunch. ‡ Get patients started with 2 prescriptions V-Go® Rx Select the appropriate V-Go option Insulin Rx U-100 fast-acting insulin* • 2 vials for V-Go 20 • 3 vials for V-Go 30 and V-Go 40 Meet patient needs with 3 V-Go options † † † Patients can push the Bolus Delivery Button only 18 times in every 24-hour period. Important Risk Information: If regular adjustments or modifications to the basal rate of insulin are required in a 24-hour period, or if the amount of insulin used at meals requires adjustments of less than 2-Unit increments, use of the V-Go Disposable Insulin Delivery Device may result in hypoglycemia. The following conditions may occur during insulin therapy with the V-Go: hypoglycemia (low blood glucose) or hyperglycemia (high blood glucose). Other adverse reactions associated with V-Go use include skin irritation from the adhesive pad or infections at the infusion site. The V-Go should be removed before any magnetic resonance imaging (MRI) testing. *A U-100 fast-acting insulin should be used with V-Go. Humalog® (insulin lispro, rDNA origin) and NovoLog® (insulin aspart, rDNA origin) have been tested by Valeritas, Inc. and found to be safe for use in V-Go. References: 1. Bode BW, et al.
