REGISTRATION INFORMATION:
The appropriate registration fee must be received by Sherri Scharff at NCIS no later than
December 18, 2014 to avoid the “Late” fee. Registration after this date will be on a space availability
basis and will be charged the late registration fee. The registration fee will be fully refundable due to
cancellation up to December 18st. Refunds cannot be issued after this date, however substitutions are
allowed. To register, complete the attached form or register online at www.Ag-Risk.org. Be sure to
fully complete the registration form with all information required.

Late Registration

Fee includes lunch, coffee breaks and meeting supplies.
NCIS Regional/State Annual Meetings are open to NCIS member company designated representatives,
RMA representatives, invited educators, and invited agent organization representatives. General
attendance by agents and adjusters is not permitted.
