Case Manager – Port Macquarie and Surrounding Areas Jo-Ann Marsham Profile I have worked with people with acute and chronic acquired brain injury for over 7 years. In this time I have developed a network of contacts through the public and private sector, seeking medical advice needed for an individual with an acquired brain injury and their family/friends peace of mind. With a person centred approach and working within the guidelines of LTCS, all aspects for the participant is considered and discussed for goal setting. Open communication and honesty is a priority for ideal coordination for rehabilitation. I am based in Port Macquarie but prepared to travel to surrounding areas. Travelling west to Tamworth, south to Taree and north to Coffs Harbour is open to discussion. This is a time of major life changes and working in this situations and hearing you and your family’s needs is what I endeavour to achieve and act upon. I am a recognised Case Manager with the Lifetime Care and Support Authority. I keep current with events and training.
