December 22, 2014 Dear Patient, We are writing to confirm for you that your Medical Team formerly based at Manet’s Quincy Medical Center location, on the 2nd floor has relocated to Manet’s location in North Quincy at 110 West Squantum Street, effective December 15, 2014. This Medical Team includes your Manet Physicians: Dr. Lily Yung, MD, Dr. Vikram Butani, MD, Dr. Karla Talledo, MD and Nurse Practitioner Tamar Siracusa, MSN, ANP-BC, WHNP-BC. Joining your medical team are the same nurses, case managers, and front desk staff that you have come to know with your care. Manet’s location at 110 West Squantum Street is just 2.6 miles from the former Manet location inside Quincy Medical Center. Our Manet North Quincy site is accessible via the Quincy Care Network Courtesy Shuttle, car, or via the MBTA, Red Line (North Quincy, 110 West Squnatum Street) stop. Parking is available at 110 West Squantum Street. Once entering the Manet location in North Quincy, you will find your Medical Team waiting for you on the first floor inside a beautifully renovated health center complete with a Pharmacy and a Vision Center to open in January 2015. Manet, at 110 West Squantum Street, will be .4 miles further on the right. (Turn right just before approaching the building onto Safford Street to enter the Manet parking lot.)
