Admissions Committee can only review applications that are complete. Your application
to the UNMC School of Allied Health Professions is a 2-step process. Both steps are
detailed in these instructions. Complete both steps and submit the $70 application fee
by 11:59 PM CST on May 1, 2015

To start the application process, a unique ID (NUID) and password are needed. If you are new to the
Nebraska College or University system, click on the "Create Your NUID" link to be assigned a universal
NUID. If you are a current or former employee or have previously applied to or attended Chadron, Peru,
or Wayne State Colleges; UNK, UNL, UNO or UNMC, use your NUID (Student ID number) and current
password to apply. If you do not know your NUID or have forgotten your password click on the "Forgot
Your Password or NUID" link.

Troubleshooting
Please follow the steps below if you have any trouble completing or submitting the application:
1. Delete your browsing history.
2. Use Internet Explorer or Firefox (if either of these isn’t working for you try the other). DO NOT use
other browsers.
Step 1: On-line Application
Complete the online UNMC application and submit the $70 application fee by 11:59 PM CST on the
deadline date. Thoroughly read all instructions in the application and complete all required and
applicable information.

a. Enter residency information. If you are a Nebraska state resident, be sure to check the box
stating, “I am a Nebraska resident,” and complete the four validation questions. If you are not a
Nebraska resident continue on to the next section.
6. Academic Term and Program
Select Professional for Academic Career
Select Term (Fall 2015) for which you are applying
Select Professional Degree Seeking for Admit Type
Select Allied Hlth Prof for Academic Program
Select Cytotechnology Satellite for Academic Plan
7. Educational History
a. High School Information – enter information about the high school from which you graduated.
i. Select the type of high school from which you graduated (i.e. US high school, GED, home
school or high school outside the US)
ii. If a US high school, search for the high school by clicking on the “High School Search”
button. Enter city and state information and click “Search”.
iii. Enter dates of attendance – “From Date” and “To Date”
b. College Information – enter information about ALL institutions you have attended
i. Select the option “College(s) Attended / Currently Attending”
ii. Search for the College / Institution by clicking on the “College Name Search” button. Enter
city and state information and click “Search”
iii. Enter dates of attendance – “From Date” and “To Date”
iv. If you have received a degree from the institution, select the degree type from the drop
down list and enter the date degree was received. If you did not receive a degree from
that institution, leave the “degree” and “degree date” box empty.
v. To add multiple institutions, select “Click here to Add another College Education
Experience.”
IMPORTANT: Enter information for ALL institutions attended, including college or universities attended
while in high school. We require official transcripts from ALL institutions that you have attended,
including all foreign transcripts.
8. Supporting Materials – Upload Current Resume and Personal Narrative.
a. Personal Narrative – Your narrative should be a maximum of 500 words. The narrative should
describe your interest in your specific discipline, particularly stressing your professional goals and
motivation for seeking this certification. Additionally, please describe your current experience in
your profession.
b. Resume/Curriculum Vitae - Your resume or curriculum vitae should include the following
information - an objective statement that addresses your interest in completing an advanced
degree and states your professional career goals. Please include all schools attended and dates
of attendance, current place of employment and all previous places of employment since
graduation from your professional program. Please indicate all certifications with current status.
9. Recommendation Details –DO NOT COMPLETE THIS PORTION OF THE ONLINE
APPLICATION. Please skip this portion of the online application.
10. Questions – Answer all of the questions.
11. Application Specific Questions – Answer the question that addresses which site you are applying
for (Carle Clinic, UC Davis, The Ohio State).
12. Admissions Application Fee – select the method of payment and follow the instructions provided.
a. Credit Card (VISA or MasterCard) – if you will be paying the application fee by credit card, please
have the credit card ready. After clicking the “Make Credit Card Payment” button, you will be
directed away from this site to make the payment. Once the payment is complete, you will be able
to return to the application to submit.
b. Waiver – Every applicant must pay the $70 application fee in order for his/her application to be
processed.
13. Confirmation – Review the statement and check the box to accept. A summary of your application
can be viewed and saved for your records. Click “SUBMIT” to complete the application.
14. Confirmation Email – A confirmation email will be emailed to your preferred email address indicating
your application has been submitted and received by the SAHP Academic and Student Affairs at
UNMC. Please be sure that you enter a valid email address because any questions about your
application and admissions decisions will be emailed to that address.
Step 2:
All materials submitted in support of your application are due by the application deadline and become the
property of the University. They cannot be returned or forwarded. Incomplete applications will not be
considered.
1. ACADEMIC & PROFESSIONAL RECOMMENDATIONS: Please submit three (3) letters of
recommendation. You can collect these letters and mail to us directly or provide your recommenders with
addressed and stamped envelopes and they can mail them to us. Three (3) recommendations are
required. Please submit a letter of recommendation from an educator (someone that instructed you),
current or former employer and a personal contact. Please make sure that your recommendations reach
us by the application deadline.
2. TRANSCRIPTS: All OFFICIAL TRANSCRIPTS must be submitted from ALL colleges/universities
attended. All transcripts must be submitted in sealed envelopes from the institution to be acceptable.
Transcripts will be verified by the registrar as original and official. If your institution offers electronic
transcripts, UNMC will accept those as well. They must be sent directly to UNMC at the email address
below.

APPLICATION STATUS: To check your application status and/or verify that we have received your application
packet, please email [email protected]. All official communication from the School of Allied Health
Professions will be sent via email. To ensure you receive all important communications, you should regularly check
the email account you provided when you applied online. If the email account to which you wish to receive
