As we navigate the road through the care continuum, the focus is on doing it safely for the patient and the provider: With increasing emphasis on successfully transitioning patients between settings, case managers and rehab nurses must attend to the details to facilitate a smooth move to the appropriate levels of care. It is essential that case managers know the common reasons for transition failure and how to intervene to reduce readmissions and post discharge problems. This session will explore problem areas in transitions, how to use the variety of care settings available, and interventions for inpatient and external case managers to use. Telephone (day) ________________________________ (evening) __________________________________ ABOUT THE SPEAKERS Patricia Quigley, PhD, MPH, ARNP, CRRN, FAAN, FAANP, Associate Director, VISN 8 Patient Safety Center of Inquiry, is both a Clinical Nurse Specialist and a Nurse Practitioner in Rehabilitation. As Associate Chief of Nursing for Research, she is also a funded researcher with the Research Center of Excellence: Maximizing Rehabilitation Outcomes, jointly funding by HSR&D and RR&D. Her contributions to patient safety, nursing and rehabilitation are evident at a national level – with emphasis on clinical practice innovations designed to promote elders’ independence and safety. She is nationally known for her program of research in patient safety, particularly in fall prevention. She has presented at national Evidence-based Fall Prevention Conferences for 11 years consecutively and is a considered a falls prevention guru . We heard Dr. Quigley speak at the national ARN conference and are both thrilled and honored that she is coming to MI-ARN to share her knowledge. Her fall prevention passion is contagious. Additionally, she provides on-going consultation to the nursing staff, quality management, and patient safety coordinators for management of complex patients at risk for falls. Donna Williams, RN, MS, CRRN is a case manager with Kaiser Foundation Rehabilitation Center, providing acute rehabilitation services. Donna is well known to ARN, having been on the national board for over twelve years, serving as its president in 2001-2001. Both her nursing and rehab career has spanned a variety of settings and areas from staff nurse, advance practice nurse, case manager, professor, consultant and educator. She has been a speaker at professional associations, health care agencies and private education providers on such topics as clinical rehabilitation, rehabilitation concepts and case management. International presentations included Scotland and Ireland. We are thrilled that Donna will be with us as we have learned so much from her presentation last October in Charlotte. Kathleen Urban, MS, BSN, CCM, CRRN is the Director of Rehabilitation Services and also the nursing patient safety officer at Garden City Hospital. She is a certified crises intervention instructor. She has been active both on the local and national boards of ARN for many years. Her passion and enthusiasm for rehabilitation nursing is only seconded by her strong commitment to safety at every level. Kathleen has presented at national conferences and has been published.
