Purpose of Addendum On May 20, 2014, Ms. Bouverette requested to add the Living Room and Bathroom on the main floor of her home to the approved child use space for her group child care home. III. Methodology 05/20/2014 Modification Request Received 05/20/2014 Inspection Completed Onsite. IV. Description of Findings and Conclusions The terms of the original license have been modified on May 22, 2012, and May 20, 2014. The group child care home is a single story modular home, with an attached finished lower level. The home is located in a quiet residential area in Bad Axe. Ms. Bouverette currently uses the finished lower level of her home and the main level Great Room for approved child use space. I inspected the first floor Living Room and Bathroom. The first floor has two remote exits for emergency evacuation. The first floor also contains operational smoke and carbon monoxide detectors. A fire extinguisher is located on the first floor of the home. The Great Room and Living Room each contain several pieces of living room furniture and a television. The area is connected to an attached kitchen. Ms. Bouverette intends to provide direct supervision to children who will be using the first level of the home. Ms. Bouverette has ample equipment and space to support the addition of the Living Room and Bathroom on the first level of the home to the approved child use space. Terms: Ms. Bouverette is approved to provide care for up to 12 children aged birth to 12 years of age. Ms. Bouverette provides full meal service. 2 V. Recommendation I recommend the addition of the Living Room and Bathroom on the first floor of the home to the approved child use space.
