Important information for participants who have a self-directed brokerage account in a Bon Secours Health System Inc. Retirement Plan. As previously announced, the Bon Secours Retirement Plans will be transitioning to Fidelity Investments as of July 28. 2014. Enclosed is information about Fidelity BrokerageLink®, including the Participant Acknowledgement Form. This form must be completed, signed, and returned to Fidelity Investments, P.O. BOX 770001, Cincinnati, OH 45277-0036 by June 20, 2014 in order to open your new Fidelity BrokerageLink® account, and to initiate the transfer of assets from your current brokerage account to Fidelity. We will begin to transfer your brokerage account as of July 24, 2014 Securities held in your brokerage account typically transfer electronically, and usually become available for trading within 5–7 business days. In some cases, securities held in your brokerage account must be manually re-registered and may take approximately six weeks to transfer; it may be possible to accomplish trades through your former custodian up until they initiate the transfer. Any outstanding orders at your prior custodian will not be transferred to Fidelity. Please be aware that the value of your securities could fluctuate during the time period when you will be unable to initiate a trade. Once your Fidelity BrokerageLink® account is open, and your assets have been transferred, you will receive more information about accessing your account, and making transactions using Fidelity BrokerageLink®. Complete the remainder of Section 1 and Sections 2 and 3. 3. Remove the pages containing the Fidelity BrokerageLink® Account Terms and Conditions and retain them for your records. 4. Sign the completed form in Section 4, make a copy of both pages of the form for your own records, and return the original to Fidelity Investments, P.O. If you would like to set up third party access, which will allow an investment advisor or other person you designate to execute trades in your account, please complete the enclosed Fidelity BrokerageLink® Limited Third Party Trading Authorization and Indemnification Form. Please note: A BrokerageLinkSM account must be automatically established for you due to the transfer of your retirement plan balances which include brokerage assets. As of July 24, 2014, the process of transferring your brokerage account assets from your Schwab Personal Choice Retirement Account (PCRA) to Fidelity Investments will begin. You will receive a welcome package once your Fidelity BrokerageLink® account is opened. Though this account is established, if you do not return the completed Fidelity BrokerageLink® Participant Acknowledgement Form, your brokerage account will be frozen for purchases. You will be able to place sale transactions only. You will not have full access to trade in your Fidelity BrokerageLink® account until the completed form has been received by Fidelity.
