〇 Insured persons in the FR Health Insurance Organization as of the last day of
April who remain insured persons as of the date they undergo their health checkups.
〇 Those who are not insured persons are eligible if their employer has instructed them
to undergo the health checkups.
〇 We ask all eligible persons to be sure to go to get their periodic health checkups.

Implementation of periodic health checkups
This fiscal year, FR Health Insurance Organization and FR Group companies who are members
of the Health Insurance Organization will once again provide periodic health checkups. The purpose of
these checkups is to give members an opportunity to check on the status of their health and to
promptly identify and treat any unforeseen or underlying conditions.
This program also provides a full range of optional exams specific to each age bracket and
gender, including screenings for cervical cancer, breast cancer, and stomach cancer.
Your employer and the Health Insurance Organization have the following requests for members
concerning the implementation of these health checkups:
・We ask that eligible persons be sure to undergo their periodic health checkups.
・If you are instructed to undergo a follow-up exam or told you need treatment, please make this a top
priority and act promptly.
・If you notice a need to improve your life habits, please review your diet and exercise practices.
・We also ask that branch managers, division managers, leaders, and other superiors set an example
in undergoing checkups themselves and in encouraging eligible persons to complete their checkups
during the checkup period.
The FR Health Insurance Organization draws on health insurance premiums paid by insured
persons and employers on a fifty-fifty basis to cover the medical care costs of members. Healthy
members generate low medical care costs, which helps keep health insurance premiums low. Let’s
work together to make the Health Insurance Organization strong and sound.
We strongly encourage members to get their annual health checkups and to treat these
checkups as opportunities to examine their own health status. Doing so will help them deepen their
understanding of life and health.

Get a health checkup and review your life habits
As busy as you are with your daily work responsibilities, you’re likely to take your health for
granted from time to time. Keep in mind that health is a key prerequisite for a rewarding life and
rewarding work. Good health lets you enjoy life and make plans for the future. Be sure to double-check
your own health by undergoing an annual health checkup. While we may not think about our health
every day, it’s a good idea to be on the lookout for any signs your body may be sending and to make
course corrections regarding unhealthy life habits.
Health checkups are important for the following reasons: (i) They help catch illnesses in their
initial stages; (ii) they help identify changes that may represent early warning signs of various illnesses;
(iii) they identify any need for early treatment; and (iv) they create opportunities to correct damaging life
habits.
In general, we rarely think about our own health. For precisely this reason, we encourage you to
set aside time once each year to consider and to care for your own body.

・ Appointments and checkups are available only during the period shown above. If you lose your eligibility for coverage by the Health Insurance Organization
before the checkup date due to retirement or other reasons, you will not be eligible for the checkup.If you do undergo a checkup after loss of eligibility, you
will be obligated to pay the entire cost of the checkup, including the portion ordinarily covered by health insurance.
・ If you went for a checkup while employed during FY2014, you cannot undergo another checkup after becoming a voluntarily and continuously insured person.

STEP5
Get your checkup.
● Go to get your checkup on the scheduled appointment date.
Bring the following items with you:

□ Specimen containers (if received from the checkup facility)
* If you made your appointment by smartphone, in place of a checkup ticket, inform the checkup facility reception

● Checkup results will be delivered from the checkup facility about one month
after the checkup.
Changing or cancelling your appointment
Contact the checkup facility to change or cancel
your appointment, then complete the procedures
under “Check, Change, or Cancel Application
Details” on KENKOBOX.

Yes. However, you must cover the entire amount of
the examination cost of optional exams other than
those specified for you. In such cases, the names
and personal cost obligations for the optional exams
will not appear on the checkup ticket issued by Ewel,
Inc. Check with the medical facility concerning
examination costs.

facility more than three days ago. Can I still ask
to have my checkup ticket issued?

I just sent my checkup ticket application form by
fax. How can I tell if it arrived?
A
Yes. Keep in mind that the less time until the
checkup date, the more likely it is that specimen
containers or other materials from the checkup
facility will not arrive in time. We encourage you
to have your checkup ticket issued as soon as
possible.

It takes roughly two to three days after a fax is
sent before we can confirm arrival.

What should I do if no available checkup
facilities are located near my store?

Handling of personal information at Ewel, Inc.
Personal information entered as part of procedures is used solely to provide the health checkup services applied for.
This information is provided to third parties such as partner medical facilities within the scope of such use.
Personal information obtained in such cases is handled in accordance with the Basic Policy on Protection of Personal
Information and the Privacy Policy provided at the Ewel, Inc. website (http://www.ewel.co.jp/privacy/index.html) and with
the policies set forth below. Users are asked to consent to these policies before submitting such information.
<Handling of personal information during use of health checkup services applied for:>
(1) To check the contents of appointments for health checkups, Ewel, Inc. provides personal information to medical
facilities. This includes the applicants’ names, addresses, telephone numbers, birthdates, and the health insurance
organizations to which they belong.
(2) Ewel, Inc. obtains all personal information concerning applicants from medical facilities, including the personal
information listed under (1) above and data on health checkup results (generally referred to hereinafter as “relevant
personal information”).
(3) Ewel, Inc. maintains and administers the relevant personal information.
(4) Ewel, Inc. provides the relevant personal information to health insurance organizations to which the applicants
belong.
(5) In certain cases, Ewel, Inc. may provide the relevant personal information to subcontractors for analysis, study, and
administration of health checkup data by the health insurance organizations to which the applicants belong.
(6) In certain cases, Ewel, Inc. or health insurance organizations to which the applicants belong may provide the
relevant personal information to firms providing health-maintenance guidance services for purposes of providing
special health guidance.
While entry of information during procedures is at the user’s discretion, note that failure to enter information or errors
in items of information entered may result in the inability to provide the health checkup services applied for. Ewel, Inc.
may also provide personal information in the form of statistical data that does not allow identification of specific
individuals to the individual health insurance organizations to which the applicants belong or to other organizations.

Handling of personal information at medical facilities
(1) For health checkups, medical facilities obtain personal information from applicants, including addresses, telephone
numbers, birthdates, and the health insurance organizations to which they belong.
(2) Medical facilities obtain the information under (1) above from Ewel, Inc. to review the content of appointments.
(3) Medical facilities provide to Ewel, Inc. all information concerning individual applicants, including personal information
and data on health checkup results.
