Information Sheet
What is Federal Impact Aid? FIA reimburses public school districts for the loss of
traditional revenue sources due to a federal presence. This impact may take the form of
dependents of military personnel or land taken off the tax rolls for military bases or public
housing. School districts find themselves responsible for educating more students with fewer
resources. Impact Aid was designed to provide money for these students. This program was
begun in 1951 and has been reauthorized 15 times since then. Impact Aid has now been
inserted into Public Law 103-382 Title VIII as part of the Elementary and Secondary
Education Act.
Many military parents of children in your school do, in fact, pay property taxes on their homes.
However, tax revenue is not generated from the buildings in which they work; they do not
generate sales tax revenue on commodities purchased on the base; and, in many cases they do
not generate revenue from vehicle licenses and fees.
We have found that there are instances where a parent is reluctant to complete some items on
the FIA Survey which is needed by the district in order to receive Impact Aid funds. Parents
may be concerned that this information request is a violation of their privacy. The following
information may be helpful to share with them:
Since Impact Aid payment is based upon certain information about each student, we ask
you to complete the card for each of your children enrolled in our schools. We assure you
that this information is the minimum required for securing available funds and does not
violate security regulations. The information you provide will be treated confidentially
with only those persons assigned to compiling and auditing this program having access to
the forms. Your cooperation in providing this important information is appreciated and
we thank you for continued support of Duval County Public Schools.
As always, our thanks go out to the individuals in the schools who work hard each year in
assisting our office with the Federal Impact Aid Assistance Program Survey.

FEDERAL IMPACT AID ASSISTANCE PROGRAM (FAP) SURVEY

The Federal Assistance Program Survey date is Monday, October 20, 2014. This date coincides
with the 45th day of enrollment for the school year and the end of the first quarter.
All Federal Impact Documents are located on our department website at: www.duvalchoice.com/forms

Survey Date. Surveys cannot be distributed before this date.
November 3November 21
Begin collecting surveys from homeroom teachers daily. Please check to see that
each survey is filled out completely and accurately. If Part ‘B’ UNIFORMED
SERVICES was completed by the parent, and the student is enrolled in a Special
Education program, then the principal must complete the VERIFICATION OF
SPECIAL EDUCATION PROGRAM box.
SURVEYS AND LETTERS DISTRUBUTED TO STUDENTS
Survey forms along with a copy of the letter to the parent or guardian should be
distributed to students for completion by the parent, guardian or person acting as the
parent. Ask that teachers request completed forms be returned immediately. Please
be as persistent as possible in retrieving completed forms.
GIFTED and HOSPITAL HOMEBOUND do not count as eligible ESE programs.

sheet with each one.

RETURNED YOUR COMPLETED PACKAGE TO THE SCHOOL CHOICE OFFICE,

ADDITIONAL SURVEYS THROUGH JANUARY 12, 2015.

Choice/Pupil Assignment Operations Office, 390-2144.

Retype the survey or letter. The information in both have been approved by the USDOE.
Complete a survey form for each student at your school who may qualify. Schools must
distribute the forms for the parent to complete.
Forget to get the principal’s signature on the ESE section if applicable.
