ILC 401(k) SAVINGS AND INVESTMENT PLAN ENROLLMENT WORKSHEET Please fill out this worksheet before you call or log in I NSTRUCTIONS TO ENROLL BY PHONE OR ONLINE 3B The Participant Information Center and Retirement Access give you two easy ways to enroll in your plan. All you need to do is complete this worksheet and then call or log in to enroll. We have provided blank spaces for each number you must enter. The system will prompt you to input your choices and then will confirm each of your entries. IMPORTANT: You will be automatically enrolled in the Plan at a 3% pre-tax contribution rate unless you specifically decline enrollment or elect to contribute at a different rate. ▪ To decline enrollment or change your contribution rate, contact MassMutual by the 20th of the month preceding your entry date. Your entry date will be the first day of the month following the day you become eligible for the plan. 4B 5B Notice of your right to have no 401(k) contributions made to the plan: You have a right NOT to have your pay reduced and contributed to the Plan. If you do not want to contribute, you must enter a 0% when you choose your contribution amount in Step 2. This must be done prior to the 20th of the month preceding your entry date. 7 When you enroll or change your contribution rate by the 20th of any given month, your new payroll deduction will begin with your first paycheck the following month. After enrolling, be sure to designate your beneficiaries on-line. If you are married, your primary beneficiary under the Plan is your spouse unless your spouse consents to your designation of a non-spouse beneficiary. If you elect a non-spouse beneficiary you must complete the Beneficiary Designation Form and return to MassMutual Retirement Services. 6B Enroll/change online 1. 2. 3. Log on to www.ilc401k.com and click on “Enroll or Access Your 401(k) Account”. Login on the Retirement Access Welcome page. Follow the online instructions Call 1-800-854-0647 if you need assistance. Service Representative are available Monday to Friday from 8 a.m. to 9 p.m. U U Enter your Social Security Number (SSN) to establish your initial User Name. Enter the month, day and year of your birthday in an eight-digit format (MMDDYYYY) to establish your initial Password. Choose a new Password Your new Online Password must be 8-12 characters containing two numbers and two letters. Special characters are supported. Your Password is case sensitive. Choose a new User Name. Your new User Name must be between 8 to 25 characters. Special characters, except for @, are supported. Your User Name is not case sensitive. You will then be prompted to set up a Password “Question and Answer.” Please note that PIN numbers for the phone and the internet password are not interchangeable. IMPORTANT: On July 1st of every year, your contribute rate will automatically increase by 1%, up to 6%, when it will then stop increasing. You can opt out of this feature at any time by calling the Participant Information Center at 1-800-854-0647. Before tax contribution: (between 1% and 25%) After tax contribution: (between 1% and 25%) ___________% U U ___________% U U Step 2 Choose Your Investments 1 The system will now prompt you to enter your investment choices. If you do not make any selections, 100% of your contributions will be automatically invested in the MFS Conservative Asset Allocation Fund. You will first hear or see that 100% of your contributions will be invested in the MFS Conservative Asset Allocation Fund. To change this you must select alternate percentages (While cycling through the fund choices on the phone system you must enter or speak “0%“if you do not wish to select a fund.) Continue until you have made all of your selections.
