UPHP Bulletin March 31, 2014 Attention: All UPHP Contracted Providers HEALTHY MICHIGAN PLAN EFFECTIVE APRIL 1, 2014 The Michigan Department of Community Health (MDCH) issued MSA Bulletin 14-11 outlining the Healthy Michigan Plan, a new category of eligibility that will begin on April 1, 2014. The benefit design of the Healthy Michigan Plan ensures beneficiary access to quality health care, encourages utilization of high-value services, and promotes adoption of healthy behaviors. All Upper Peninsula Health Plan (UPHP) contracted providers are automatically providers for the Healthy Michigan Plan. Beneficiaries in the Adult Benefits Waiver program will be transitioned to the Healthy Michigan Plan effective April 1, 2014. Please be sure to verify plan eligibility in the CHAMPS system. Eligibility Eligibility for the Healthy Michigan Plan is determined through the Modified Adjusted Gross Income methodology, coordinated through Department of Human Services. The Healthy Michigan Plan provides health care for persons who: Are age 19-64 years Have income at or below 133% of the federal poverty level Do not qualify for or are not enrolled in Medicare Do not qualify for or are not enrolled in other Medicaid programs Are not pregnant at the time of application Are residents of the State of Michigan The beneficiary will receive a new UPHP ID card upon enrollment and should present both the UPHP ID card as well as the green mihealth card at every appointment. Please use the Community Health Automated Medicaid Processing System (CHAMPS) to verify eligibility; members active with UPHP Healthy Michigan Plan will be identified as MA-HMP-MC (Healthy Michigan Plan Managed Care) in CHAMPS. Benefits The Healthy Michigan Plan covers the same benefits as Medicaid along with additional benefits. Dental services will be covered by this plan as well; UPHP has partnered with Delta Dental of Michigan to provide members with dental coverage. Beneficiaries enrolled in Healthy Michigan Plan will NOT be responsible for co-pays at the pointof-service by provider offices and pharmacies as long as the service/prescription is covered by UPHP.
