JOB POST 3/4/14 LVN, Fresno Outpatient EMPLOYMENT STATUS: LOCATION: Per Diem, Non Exempt Hinds Hospice – Fresno, California POSITION SUMMARY As a member of the interdisciplinary team, the LVN under the direction and supervision of the Director of Patient Care Services, Clinical Team Leader, or a registered nurse, provides direct patient care to patients in their home as needed. MINIMUM QUALIFICATIONS: Current LVN Licensure, CPR, recent physical and TB, Tdap, MMR, Varicella, Hep B Immunizations. One year Acute Care experience, previous Home Health/Hospice experience preferred. Demonstrated ability to work with health team personnel. Possess reliable transportation and have a valid California driver’s license. Show proof of auto insurance, DMV printout and current auto registration. Ability to meet the physical demands of the job (i.e., Push 50 Lbs., Pull 50 Lbs., Lift/Carry 25 Lbs.)
