Being very humble in his approach to life, Javier doesn’t seek out recognition for the many things he does that are over and above what is expected of him. His actions, commitment, and dedication speak to the type of person that he is and commands the respect of the entire NHR team. Javier is an integral part of the NHR team and sets the standard for taking a safety first approach to everything he does. Javier Delgado Painter NORTH HOUSTON ROSSLYN SHOP HOUSTON, TEXAS Javier has been with Exterran for two years. There are few associates who take as much pride in their work and their workspaces as Javier. During his tenure, Javier has participated in and led improvement activities that have significantly improved the safety and productivity of his department. After participating in improvement activities for other work areas, Javier utilized what he had learned to improve his own work area. The Paint Department is a multi-shift operation. Javier is one of six people who are charged with the operation of the blast equipment at NHR. Javier created a standard operating procedure and an emergency shut-down procedure for the blast equipment. After having the procedures reviewed for accuracy, he posted the procedures in the areas closest to the operation. Having already gained the respect of his peers, Javier trained the others in his department on the use of the procedure and how using the visual aid at the work station would help them refresh their own memory and provide a tool for training newly assigned personnel. 2 “Safety is important to me so I can go back home to my family and see them healthy. It’s about making sure I do my job safely and am there for my family.” Manufacturing Process & Treating immediately stopped the job and instructed the Contractor to red tape the area and fill out a JHA. The appropriate personnel responsible for over-seeing the contractors were then retrained. Four months earlier, Scott stopped the job again when he noticed two employees using a lug that was only tack welded as an anchor point for their safety harness. Scott made the employees weld out the lug before they continued working; then he took pictures and used them the following day during his department’s morning safety tailgate meeting. Scott Hester Master Welder BROKEN ARROW, OKLAHOMA Scott has been with Exterran for two years. Scott has been named safety champion of the month twice at his facility and was also named second and third quarter safety champion. Scott divides his volunteer time to support the “Veteran & Wounded Warrior Freedom Boat” program, where he provides a way for wounded veterans to relax and have fun by taking them out on fishing outings with their families and the other half of his time supporting the Children’s Burn Camp where he teaches the kids how to fish and takes them on boat rides. Last year, Scott and his veteran partner won the inaugural tournament by catching the heaviest stringer of fish and largest bass. In his spare time, he likes to hunt, fish, and coaches little league baseball & football. Scott and his wife Erin live in McCurtain, Oklahoma, and they have four children: Alex, Leah, Sayre, and Landon. Scott Hester has been working as a Master Welder in the Tower Shop and filling in as a lead when needed. Scott is an outstanding employee and has demonstrated the highest level of safety leadership. He is actively involved in process improvement activities such as Lean 5S and TPM. Scott was an integral part of helping the shop to win the Lean 5S Safety champion award two months in a row. Scott’s safety awareness has been ingrained in him from a very young age. When he was about 11, his father was involved in a severe industrial accident which hospitalized him for 30 days and confined him to a wheelchair for eight months. His father instilled in him that safety comes first. Every day, Scott goes to work with a positive attitude and looks out for his colleagues. While Scott was working in the Tower Shop, he noticed that a contractor was doing a load test on one of the overhead cranes. They were lifting a 50,000 lb. weight while employees were working within close proximity of where the lift was being made. Scott realized that the area was not properly cordoned off, and there wasn’t a JHA present in the area. He “Safety is important to me because my family and my coworkers’ families depend on us and need us to come home every night.” 3 Manufacturing Production Equipment In addition the council agreed that this hazard was difficult to recognize and that Marshall demonstrated observation skills exceeding most of his peers. Marshall was subsequently chosen as the monthly, quarterly and annual safety champion. Marshall’s actions allowed Broussard Manufacturing to acquire the correct materials in order to install a safe working system for the shrink wrap operation. Marshall always takes great pride in doing things the safest way possible. He is seen as an HSE leader on the shop floor. Marshall is married to Rayna and has two step sons and one daughter between the ages of 16 and 30. Following Marshall’s lead, his children enjoy outdoor activities as well. Marshall Romero Sr. Welder/Fitter BROUSSARD, LOUISIANA Marshall Romero has been with Exterran for eight years. In January 2013, Broussard Manufacturing acquired the equipment and materials to shrink wrap pallets using shrink bags and a flame gun. This method was adopted due to the safety issues involved with wrapping pallets with stretchable cellophane rolls by hand. During an installation of a hose and reel, Marshall Romero observed the installation operation and decided to check the hose and reel service rating. A quick investigation determined that the hose was not rated for hydrocarbon gas service and Marshall initiated a “stop the job” action. His prior training had stressed that he had the authority and obligation to stop any job that posed unacceptable risk. Clearly, transmission of hydrocarbon gas through an improperly rated hose could potentially lead to deterioration of the hose, thereby causing a gas leak and possibly a fire or explosion, putting people and property at high risk. The safety council determined that Marshall’s intuition and quick action to investigate the hose potentially saved the lives of Exterran employees and serious property damages. 4 “Without safety, we do not go home to our families and we are not supporting our company in saving money.” North America Operations Central/Northeast normal task we would expect of an Exterran FST, this specific situation had huge financial implications for the customer. The customer had to shut down a complete processing facility, and all associated work at that facility, while it took over four hours to repair the reported leak. Recently, there was a contractor in the AMS shop working on an overhead door. Barry noticed they were operating a scissor lift without fall protection. He asked the contractor standing on the floor why they did not have fall protection. The contractor responded and told Barry “Not to look.” Barry immediately stopped the job, asked the contractor to return to the safe ground level, and to discontinue working. The contractor was asked to leave and not return to the facility. Barry Hoffman Field Service Technician MID CONTINENT – PAMPA, TEXAS By using his more than 20 years of experience, and the training he has received at Exterran, Barry has made a huge difference in the very short time that he has been with the company. Barry has lived in Borger, Texas, for 40+ years and is married to Charlotte. He has two kids and three grandchildren (one on the way). Barry enjoys showing off his 1941 Ford two door sedan at car shows in his spare time. Barry has been with Exterran for four months. The Central/Northeast Region is proud to have Barry Hoffman, Field Service Technician in Pampa, Texas, as its 2013 Safety Champion. Barry has already made quite an impression on all of those around him. Barry was nominated for Safety Champion by his Manager, Joe Crump, for stopping the job not once, but twice! Barry recently caught a potentially dangerous situation during an engine swing and stopped the job so it could be corrected. The Exterran crew arrived on location and followed the standard pre-job safety checklist. They completed their JHA and LOTO, then began the task at hand: swinging the engine. The following morning when they returned to complete the assignment, they were going through the JHA and verifying LOTO was still in order, when Barry heard a whistling sound on the discharge side check valve. This valve is located downstream of the isolation valve. Barry investigated the noise and identified a gas leak at which point he used his stop work authority to halt this potentially dangerous situation. Barry notified the customer and they came out right away and repaired the leak. While this may seem like the “I know what it is like to be hurt on the job, the pain, suffering, loss of wages, the hardship on family and employer. Safety starts with me!” 5 North America Operations Gulf Coast tied into the bullet tank at a different location. He noticed that a vent line also needed to be added to the top of the tank with a back pressure valve so that excess pressure would be vented as needed. In addition, he saw that the tubing going to the ESD valves was getting supply upstream of the ESD station, essentially bypassing the ESD station, which would keep valves open during an emergency. He notified the Baetz Facility foreman of the safety issues and they were addressed by Swift Energy. Monte Moeckel Senior Field Service Technician SOUTH TEXAS – CORPUS CHRISTI, TEXAS Monte has been with Exterran for almost 19 years. Monte works for the PEQ group in the South Texas Business Unit of the Gulf Coast Region. As a Senior Field Service Tech for PEQ, he works closely with customers and enjoys the camaraderie of his coworkers. He has been called upon to mentor short service employees and relishes in sharing his wealth of knowledge with the future of this company. Monte has never hesitated to “Stop the Job” on location when safety is a concern. On July 18, 2013, Monte arrived onsite at Swift Energy – Baetz Facility in Cotulla, Texas to start up a 1,000 barrel a day oil stabilizer. While doing some preliminary checks, Monte was tracing down flow lines in and out of the oil stabilizer, fuel gas lines, and dump lines to the bullet tank. He wanted to be sure that all valves were in their proper position. He discovered that the piping, coming from the oil stabilizer and the JT skid to the bullet tank, was tied into the same line that tanker trucks used to transfer NGL from the bullet tank. A blow case dump from the oil stabilizer and JT skid could cause a surge of pressure during a transfer to a tanker truck and be a hazard to the driver. The dump line should have been 6 “I have the right to stop the job when something isn’t right to make sure everyone goes home safe.” North America Operations Southwest North Louisiana Team Arklatex – Arcadia, Louisiana Several years ago, the Southwest Region began focusing on team safety, believing that we could foster a sense of responsibility for each other within our designated work groups. This effort is readily apparent among most of our teams, which typically comprise a Compression Services service area, AMS group, or shop team. In order to qualify for consideration as a Southwest Region Quarterly Champion, teams must have zero OSHA Recordable Incidents, First Aids, Preventable Vehicle Accidents, Non-Preventable Vehicle Accidents, Environmental Violations, Spills or Fires during the quarter. To qualify as a candidate for recognition as the Southwest Region Annual Champion, the team must have gone all year without any disqualifying HSE events. The Southwest Region is proud to announce the Arklatex Business Unit’s North Louisiana service team, led by Todd Causey, as its 2013 Annual Safety Champion. They have also demonstrated “Stop the Job” authority on many different occasions, but most notably at a high H2S (a potentially toxic gas) location. Exterran was uncomfortable with the level of commitment from the customer to work safely and mitigate the hazards associated with this facility. After exercising their obligation to “Stop the Job” on numerous occasions, Todd brought his concerns to management. In essence, Todd and his team were saying that they had tried holding the customer accountable to exercise reasonable precautions to achieve a safe workplace but, when that proved inadequate, they were now holding Exterran management accountable. BU senior management addressed the issue with the customer and validated the concerns expressed by Todd and his team. Despite this being a highly profitable application for Exterran, the decision was made to discontinue operations at the site and terminate our contract. One of Exterran’s customers commented to the Business Unit Director that the North Louisiana service team “truly embraces the idea of working safely, not just doing so to be compliant with regulations.” The customer added, “We have a way to go to get our culture to the level that Exterran consistently demonstrates.” “The Southwest Region is fortunate to have this level of safety commitment and safety leadership!” The North Louisiana service team prides itself on having built a safety culture that is embraced by each member of the group. Since Todd became a Service Manager in 2007, those working under his supervision have yet to incur an OSHA recordable event. They lead the Arklatex BU in Behavior Based Safety observation card submittal, action items and closure rates. 7 North America Operations West Thomas Montoya is a true advocate for safety. Thomas has stopped the job on several occasions to either ask because he is unsure of something or to simply add additional safety measures to ensure that we are minimizing our risks daily. Thomas makes the right decision every time he starts his day to do the right thing when it comes to safety. Thomas Montoya Field Service Technician SOUTHERN ROCKIES – FARMINGTON, NM Thomas has been with Exterran for ten months. On the way home one evening near Mansanars Canyon, employees were following a truck and noticed it make a sudden lane change. An Exterran co-worker slowed down when he noticed a large bolder and other debris in his lane which was also in one of the blind curves. The employee drove around the debris with caution, and continued driving downhill. Thomas Montoya reached the debris in the roadway and safely pulled over. Thomas called his co-worker to advise him that they needed to do something about that situation because it was getting dark and the high volume of traffic was creating very unsafe driving conditions. Thomas’s co-worker safely drove back up to the rock slide area. Thomas dialed 911 to report the rock slide. Thomas then directed his co-worker to go downhill to place safety triangles in the road and direct traffic, while Thomas directed traffic to slow down on the uphill side. Employees directed traffic until the police arrived. About an hour later the Highway Department arrived and cleared the roadway. Thomas was told by the State Police officer that his safe actions may have prevented a very serious accident. 8 “I feel safety is important in all aspects of life, both at work and at home. We think about all hazards that could be involved. If we are not sure, we feel confident in calling our safety lead.” North America Operations AMS Product Line Kay’s truck is always organized and clean with extinguishers and other gear permanently affixed inside the cab for safety in case of an accident. Kay always plans long trips prior to starting in order to keep his daily DOT “Hours of Operation” correct, and prevent fatigue. Last year, Kay drove 90,000 miles without an incident. Kay’s actions are a reflection of a culture of safety at the McPherson location. Kenneth “Kay” Hewitt Senior Driver FLEET SHOP – MCPHERSON, KANSAS Kay has been with Exterran for 12 years. Kay is the employee who has handled all of the DOT concerns for the McPherson location for the last several years. The safety records for all DOT related matters have been problem free since Kay has taken over this duty. The duties include managing individual DOT medical cards, certificates of violations, DQ files, DOT vehicle inspections, apportion tags, quarterly IFTA reports, log book entries and hours of operation being kept current for both part time employees and full time employees. “Safety is close to home. To return to your family at the end of a work day is the most important aspect of safety. I do not speed through a project, but do it safely and correct the first time.” Kay is very safety minded and presents himself in a professional manner. As an example, Kay shut down a 5th wheel trailer which was being loaded with heavy equipment prior to a long round trip to Michigan from Kansas. Upon the pre-trip inspection, the trailer was found to have a wheel seal that was leaking oil and faulty clearance lights. This caused a delay in the start time for the trip to Michigan, but a wheel locking up in frigid weather in the winter or a collision with another motorist because of bad lighting would and could have been a dangerous situation. 9 I am pleased to recognize the outstanding achievements of our 2013 safety champions.
