Beyond PHS Guidelines The purpose of the Beyond PHS program is to partner with students, parents, and Linn Benton Community College (LBCC) to provide a supported bridge between Philomath High School (PHS) and the student's early college experience. The program is designed to smooth the transition between the two settings and increase the success of our students as they transition to college. It is also hoped that through Beyond PHS more Philomath High School graduates will see college as a realistic possibility. Support with the costs of tuition and books, as well as with registration, orientation, appropriate course selection and monitoring of academic progress is provided through the program. Program Guidelines • Students must meet the following criteria: o Are (or intend to become) enrolled at Philomath High School as a fulltime student (a minimum of 9 credits at LBCC) o Are or will be 16 by the start of LBCC term to be enrolled o Complete Beyond PHS enrollment process (paperwork, placement tests, etc.) prior to deadlines o Complete a personal educational plan and profile (EPP) that includes the student’s post-high school educational goal. The plan must include a college certificate or degree, which the student will pursue. This plan will include an outline of specific courses that Beyond PHS will support, in order to help the student meet that goal. The plan must be approved by your PHS counselor and LBCC staff as being realistic and appropriate for the student's placement level. o Are pursuing but have not yet earned a Philomath High School diploma including Essential Skills requirements for a diploma. o Will meet the PHS graduation requirements for CASE. (Students who are enrolling in Beyond PHS at age 16 and were previously enrolled as a home school student prior to Beyond PHS enrollment, have the option to meet this requirements by taking both, WR123 English Composition: Research, and HD 208 Career/Life Planning, and by completing the Career Related Learning Standards (CRLS) and presenting the information to a panel at the high school.) o All LBCC course work taken to satisfy diploma requirements must be completed by winter term in the planned year of graduation to be eligible to walk at graduation. If there are still credits needed to graduate, students may take courses at PHS during spring term (trimester 3) to complete diploma requirements in order to walk at graduation. (Spring term grades for LBCC classes are not available within the time frame needed, that is, before PHS graduation deadlines.) • Students must take all required state of Oregon tests and as required by Philomath High School. • Students may take classes at Linn Benton Community College (LBCC), at any of the campuses (Albany, Lebanon, Corvallis, or Sweet Home). 1/24/2014 • Students participating in the program must attend all required meetings and orientations. • Students must follow LBCC guidelines including registration processes, meeting deadlines, and seeking assistance for the many services available to students. • "Eligible courses" are defined as: o Credit bearing courses that are outlined on the student's Educational Plan and Profile (EPP) and/or LBCC graduation worksheet. o Beyond PHS will not pay for classes that are not included in the student's EPP/LBCC graduation plan without prior approval by PHS counselor. • Beyond PHS will NOT pay for classes at LBCC that are listed as having an "Internet", "Self-Directed", or "Lab" format. Any exception to this rule must be made with the approval of Larry Anderson at LBCC. • Beyond PHS will pay the tuition for LBCC classes that fall within the plan described above, up to a maximum of twelve credits per term. Students enrolled in both PHS and LBCC courses may take a maximum of 5 classes between the two schools. Students are welcome, at their own expense, to take classes that do not meet Beyond PHS eligibility requirements. Courses are only paid for by Beyond PHS during the school year (fall, winter, spring). • Books will be purchased by Beyond PHS (up to $400 per term). PLEASE NOTE: **At the end of the term students must participate in LBCC's book buy back program in order to remain eligible for future terms. If LBCC does not take your book back, please return the book to PHS. • Some classes require specialty equipment or supplies; these supplies are at the student's expense. Beyond PHS will not pay for consumables (such as art supplies), music lab fees (private lessons), or fees associated with special classes (such as scuba diving or software). • Students are discouraged from working full time or participating in some types of extra curricular activities that demand a great deal of time (e.g. competitive sports) if they are taking a full time college class load. • Students must maintain at least a 2.0 in order to remain eligible for the program. Students will be asked once per term to have their instructors complete progress reports and return them to Larry Anderson by the deadline. Students are required to complete progress reports! A second progress report may be requested if the first report indicates a need. The progress report will be shared with their PHS counselor. • Beyond PHS is a program that will change and evolve over time. Students will be expected to keep up to date and follow all future program changes. Beyond PHS program changes will be communicated through e-mail with students. Students must provide an active email address that they will check at least weekly. • Students/families may receive a 1098T form from LBCC attesting to the amount that has been spent on the student's tuition. Students and families are advised that they should only claim the amount that they themselves actually paid for tuition on their taxes and not include the amount paid by Beyond PHS. 1/24/2014 Beyond PHS Student Things to do List: Step 1: Meet with counselor for referral and paperwork packet. Step 2: Provide name, phone #, and email address to your counselor. All communications from Beyond PHS and LBCC program liaisons will go through this email address that you provide. Please use an email address that you will check at least twice a week. Sign up for an LBCC ID # if you do not already have one. (Instructions are included in the packet.) Step 3: Return paperwork packet to your counselor. Please do not leave paperwork if counselor is not there or unavailable. Paperwork due by _______________ Step 4: Take placement testing. The following steps are mandatory: o Complete the RoadRunner Entry (an online introduction to LBCC) and successfully pass the quiz in order to sign up for a Registration Session. LBCC will contact you via email with instructions on choosing classes and what paperwork you need to bring to your registration session. Go to a mandatory Registration Session at LBCC. LBCC requires you to include a Destination Graduation class in your schedule during your first term of classes. Go to mandatory Advising Appointment at PHS with your Counselor. Appointment will be made when placement testing is complete. I understand that it is the student’s responsibility to keep up to date and follow all future program changes. Beyond PHS program changes will be communicated through e-mail with students. _____ I must be enrolled as a Philomath High School student. If I am transitioning in from home school, I must contact LBLESD to change my enrollment status. _____ I agree to check the email address that I have provided to Beyond PHS a MINIMUM of twice a week. I will keep the Beyond PHS liaisons informed about any changes in my email address as well as changes in my home address and phone number. I understand Beyond PHS and LBCC will use my email address to communicate with me about Beyond PHS updates and other important information about the program. ______ I understand that there are mandatory meetings that I must attend with PHS counselor and LBCC advisor. ______ I agree to take any reading, writing, and math courses prescribed by LBCC (based on my LBCC placement test scores) as being necessary to ensure my success in the program, as well as Destination Graduation. ______ I agree to complete the LBCC Roadrunner Entry and attend a LBCC Registration Session that may be held on the LBCC Albany campus. Destination Graduation must be taken during my first scheduled term of classes with LBCC. If I do not fulfill these requirements, I will not be eligible for the Beyond PHS program. ______ I agree to take all state tests required by Philomath High School and/or the State of Oregon. ______When registering for classes at LBCC, I agree to follow the Educational Plan and Profile (EPP) that I negotiate with Beyond PHS & LBCC. I understand that I will be responsible for the tuition and book costs of any LBCC class that I take which is not part of my approved EPP. ______ Beyond PHS will NOT pay for classes at LBCC that are listed as having an “Internet”, “SelfDirected, or “Lab” format. Any exception to this rule can only be made with the approval of Larry Anderson at LBCC. ______ Students sometimes make changes in their education and career goals. I understand that I can renegotiate my EPP with Beyond PHS & LBCC if my goals change. ______ I understand that successfully completing college courses requires a great deal of time spent studying. I understand that both Beyond PHS and LBCC discourage full time employment or participation in sports programs or other demanding extra-curricular activities while enrolled in a full time college schedule. ______If I am involved in athletics and/or other school activities, I understand that I must meet the requirements set by the Oregon School Activities Association (OSAA) and the Philomath School District, both during the term that I wish to be eligible to compete/participate and the term previous to it. ______ I understand that Larry Anderson is my main point of contact at LBCC. I agree to contact my course instructor, Larry Anderson or Beyond PHS staff if I am struggling with my classes in any way. I understand that LBCC has a great deal of services to support my learning, including the Learning Center and individual tutoring. ______ I understand that I need approval from Beyond PHS Staff before dropping an LBCC class after the add/drop deadline that occurs two weeks following the start of a term. I understand that doing so without this approval may endanger my eligibility to participate in Beyond PHS in the future. ______ I understand that I will need to maintain a 2.0 GPA in order to remain in the Beyond PHS program and that I have to be making satisfactory progress toward Philomath High School’s Diploma while enrolled in the program, in accordance with my Educational Plan and profile (EPP). 01/24/2014 ______ I understand that I must be a full-time student – either taking a minimum of 9 credits at LBCC or a combination of classes at LBCC and PHS. ______ I understand that the student, parent, Beyond PHS or LBCC, can terminate the placement, without cause, if it is deemed inappropriate for the student. ______ I am willing to provide completed progress reports as required by the program and return them to Larry Anderson by the deadlines that he gives me. I understand that I must turn in my progress reports in order to remain eligible for the program. ______ I understand that I am responsible for maintaining contact with my CASE advisory teacher at PHS in order to document my progress and to remain informed about senior class announcements and deadlines. I understand if I am eligible to take classes in lieu of a CASE project, I must initiate and maintain contact with the PHS Counselor in order to complete any Essential Skills, CRLS and/or testing requirements. _____ I understand that if I receive a 1098T from the college that I am attending, I may only claim the amount I paid in tuition on my taxes. _____ I understand that I must participate in LBCC’s book buy back program in order to remain eligible for future terms. _____ I understand that I must have all my LBCC courses completed by the end of winter term senior year to participate in graduation ceremonies at PHS. LBCC EXPECTATIONS _____ I understand that: I will be enrolling into an adult environment and that I am assuming an obligation to conduct myself in a manner compatible with the functions of the college, as an educational institution within a civilized society. There are significant differences between the high school atmosphere and the college atmosphere. For example: The student population at community college consists primarily of adults who neither need, nor receive supervision from the staff. The community college staff expects mature, adult-like behavior of all students and do not tolerate inappropriate behavior. The community college does not provide follow-up on nonattendance; students are expected to attend on a regular basis. Most students find that the pace in the classroom is significantly accelerated compared to that of high school. Upon entering the college, students assume responsibility for their actions and for their successes. This includes following all registration processes, meeting deadlines, and seeking assistance for the many services available to students. You may show photo ID at any LBCC Center to retrieve your LBCC ID number. Please follow these instructions to obtain an LBCC ID number (you need to have your social security number ): • Go to LBCC website: http://www.linnbenton.edu/ • Hover over “Future Students” (far left hand side) • Click on “Make It Official” • Click on “Application Procedure” • Click on the box – “Take a Class for Training or Fun” (this wording doesn’t really keep high school students in mind but this is the right form place to go.) • Use up to 9 letters/numbers for log in name - it doesn't matter what you use, the only reason to remember it is if you don't complete the process in one setting (but it is a short process!) • Use your date of birth as your PIN - this is important... two digits for month, two digits for day, two digits for year. (example: if your birth date is January 10, 1993 = 011093) • Admission term – you may only have one option to choose from, it doesn't matter, it is a place holder, but you do need to choose a term. • Use your full name - not a nickname or shortened version of your name. In the future, you may need to show photo ID to retrieve student information - and your ID needs to match what you put down as your name. • Follow remaining instructions with address, phone number, etc. Once all four check list items are complete. (Name, Address - permanent and mailing, Personal Information). Click on "Application is Complete". • An LBCC student ID number beginning X00 ...should appear. Please write this number down -and put it on your Campus High School Programs Form.
