"Inspiring Passion and Teaching the Skills of the Beautiful Game Since 1994" Magic Moves is a soccer skills training program developed to help players increase their joy in the game through a structured approach to the fundamental and creative elements of soccer technique and movement. We strive to complement and support the efforts of club and high school coaches providing players with the best opportunities to improve their ball handling ability and confidence. We are thrilled to initiate our partnership with Madison 56ers in 2015 with our winter training sessions, offering the following. 8 hours of intensive training with an individual focus to increase ability and enjoyment. PARTICPATING IN THE TRAINING PROGRAM.
PLEASE READ ALL TERMS AND CONDITIONS BEFORE SIGNING THIS FORM. ONCE THE
PARTIES BELOW HAVE SIGNED THE DOCUMENT, IT IS A BINDING CONTRACT
THIS AGREEMENT (“Agreement”) is dated and in effect as of the latest signature below between the parent
or legal guardian of the athlete (hereinafter referred to as “Client”) and Robert Kehoe & Associates, LLC
(hereinafter referred to as “Company”).
1.
The services provided are solely those which are identified on Page 1 of this Agreement. Client
understands that all of the agreements between the parties are set forth in writing in this document. No other
terms, conditions or modifications are binding. No oral promises by any owner, representative, employee,
volunteer or other agent of Company are binding.
2.
The Client hereby agrees to waive and release any and all owners, representatives, employees,
volunteers or other agents of Robert Kehoe & Associates, LLC from any and all liabilities, claims, demands,
losses and costs associated with, or related to the Magic Moves Training program. This shall include liabilities
arising out of bodily injury to, or death of, any athlete participating in, or related in any way, to the Magic
Moves Training program.
3.
The fee for the Magic Moves Soccer Training program is $140. This fee shall be due upon receipt of
this signed form. This fee is nonrefundable.
4.
In the event of poor weather conditions or unforeseeable circumstances, Company, in its sole
discretion, may reschedule the Magic Moves Training program.
5.
In making and performing this Agreement, the Company shall act at all times as an independent
contractor. Nothing related to this Agreement shall be deemed or construed to create any employment, joint
venture, partnership, franchise, trust, or agency relationship between the Company and Client for any purpose.
6.
Client recognizes that Company may record video or take photos of the Magic Moves Training
Program. Client hereby agrees that Company has the right to display, reproduce and distribute any video or
photo created from the Magic Moves Training program. Client hereby assigns to Company all interests or
intellectual property rights the Client may have in any video or photo from the Magic Moves Training
Program. Client agrees that Company will be the sole owner of all patents, copyrights, trademarks, trade
secretes and other intellectual property rights pertaining to these videos or photos.
