Monroe County’s Building Healthy Communities Coalition will be conducting a comprehensive Community Health Assessment for Monroe County. The goal of the project is to identify
health needs in our community and develop a Community Health Improvement Plan to address
these needs. A Community Health Assessment looks at multiple data sources to determine areas
of need as well as strengths to assist local communities in allocating resources and services.
One major component of the assessment project is an adult and youth health survey to help
identify key health needs and concerns in Monroe County. The adult surveys will be conducted
through U.S. mail and the youth surveys will be handled within schools.
Hundreds of Monroe County residents will soon receive notification that they have been randomly selected to complete an anonymous and confidential survey. As stated, the survey is
anonymous and answers can not be linked to individuals. The results of the assessment will
help many local agencies to address areas of priority for health related needs as well as create
awareness of those needs within the community. The survey takes approximately 20 minutes to
complete, so if you are a chosen participant, please consider filling it out. Your input is very
valuable to the success of the project and the future health of Monroe County.
Monroe County Board of Commissioners Chairman J. Henry Lievens noted, “The partnership
between the county’s Public Health Department and Promedica Monroe Regional Hospital laid
the foundation for enabling this project to proceed. It is very important to recognize the value of
collaboration and resource sharing between public and private agencies as they work together to
deliver the best for our citizens.”
The Building Healthy Communities Coalition consists of many agencies in Monroe County that
provide services, programs and resources for our citizens and share the vision of empowering
Monroe County residents to live healthy lifestyles. The coalition meets regularly and they continue working toward their mission of improving the health of county residents by promoting
physical activity, healthy eating and safe environments. When this mission is achieved, the
community wins and most importantly, individuals improve their quality of living through better personal health.

“The coalition is excited to perform this health assessment project in Monroe County; the last
one completed was in the late 1990’s and many health factors have changed since then” stated
Jamie Dean, Chairperson for the coalition. “The importance of understanding the needs of the
community is critical to best allocating limited resources for the provision of services”, she
added.
