We thank you for your patience! 2 General Information GENERAL INFORMATION Registration Forms All forms must be completed on both sides prior to participation. Incomplete forms will be returned. Pre-registration is required for all Link Leisure activities. *ON SITE REGISTRATIONS WILL NOT BE ACCEPTED!!* Information collected from registration forms is used to enhance individual program needs, ensure expedient medical attention if required, and demonstrate merits of leisure programming to funding sources. Please be sure to complete both the front and back of the registration form. Incomplete forms will be returned to sender to be completed before processing. Spinners, Club Travel, Community Art Connection Registrations for these activities will be accepted according to individual needs in the following priority on a first come/first served basis: 1. Individuals living alone, with parents or independent of scheduled support 2. Individuals with 2-30 hours per week of SCL support. 3. Individuals with 31-90 hours per week of SCL support.* 4. Individuals with 91-148 hours per week of SCL support.* *Support verification may be required Consent and Waiver Forms EVERYONE registering for activities must complete these forms. All consent and waiver forms received during this brochure period will be good through December 31, 2015. Registrants will be ineligible for participation without these forms on file. Confirmation for Participation Activity Confirmation Invoices will be sent to you once your registration is processed. The invoice includes the total amount of payment for activities, payable to Link Leisure Services. (This includes late payments from prior brochures.) PAYMENTS ARE DUE JANUARY 30TH, 2015 Waiting Lists Waiting lists will be established for each activity. You will not be responsible for payment of activities that you are on the waiting list for until you receive confirmation for participation. Staffing Assistance If 3 or more individuals from the same residence are attending an activity, accompanying staff must remain at the activity site. Staff is also required for any participant requiring special assistance and/or one-on-one support. If staff will be accompanying a participant to a ticketed event the staff must pay the fee by the deadline. 3 General Information REGISTRATION PROCESSING BEGINS: DECEMBER 5TH, 2014 Program Times We ask that you please adhere to times posted for activities. Activities are often scheduled back to back and we are unable to provide supervision prior to or after an activity. For the safety of your child or consumer, PLEASE DO NOT drop off any earlier than 5 minutes prior to an activity and pick up promptly at the scheduled conclusion. Please make sure transportation is arranged prior to the activity. Telephones may not be available at each activity. Program Fees Program registration fees vary for each program. Individuals will be charged for each activity or program that they are registered and confirmed for, according to the registration fees indicated in the Leisure Times brochure. Send no money with your registration, you will be billed! Payment schedules may be arranged on an individual basis by contacting the Link Leisure Department. If staff will be accompanying a participant to a ticketed event the staff must pay by the following date. PAYMENTS ARE DUE JANUARY 30TH, 2015 Transportation Individuals who register for Spinners, Club Travel and or Special Events and who receive 0-30 hours of SCL support, live alone or have no means of transportation, except mass transit, are eligible for transportation. Contact the Leisure Department at 2628888 when you receive your confirmation letter to request transportation. Transportation services are limited and available on a first come first served basis. The cost for transportation is $8 round trip or $5 one-way. Once transportation is confirmed you are responsible for payment unless proper cancellation notice is given. In the event an unconfirmed/unscheduled ride is provided for take home, you will be billed $25.00 for the service. Cancellation Policy If a participant is unable to attend any program activity, they must contact the Leisure Department at 262-8888 no later than NOON THE BUSINESS DAY PRIOR to the activity. If proper notice is not received, the participant risks losing all pre-paid fees for the activity. Failure to give proper notice of cancellations 2 times in the same brochure period will result in removal of the individual’s name from all scheduled activities in that brochure period. Registration fees in this situation are non-refundable! We do not receive messages left on the Leisure Hotline; you must dial a direct extension. VOLUNTEER OPPORTUNITIES Volunteer opportunities are identified throughout the brochure. No matter what your special talents, interests and schedules are, you can help change someone’s life at Link. Helping Hands Volunteer Program Volunteering helps the world go round – at least for non-profit and communitybased programs and agencies! It is, however, hard to know where to start. That’s where we come in! In this volunteer program, we will do a project and deliver the project to the designated agency or program. Upon delivery, we will get a short tour and discussion about other volunteer opportunities within that agency or program. In this brochure we will: •Make greeting cards (birthday, Valentine’s Day, just because) to give to seniors at Arbor Springs • Fill toiletry bags and deliver to Central Iowa Shelters and Services for homeless men and women By volunteering a small amount of your personal time and energy, you can help make a difference in someone else’s life, as well as in your community! Sometimes we feel cranky, down, and tired. Take some time to rejuvenate your mind in this exciting new program! We will explore some different techniques related to relaxation and coping – journaling, anger management, pet therapy, and meditation and mantras. You will leave feeling free, open, and happy! And let’s start from scratch! On night one of this two-part, super fun program, we are going to sand and stain our very own extra-large Jenga blocks – don’t forget to wear an old t-shirt and pants; we will also come up with some questions and phrases to write on the Jenga blocks, making the game a great conversation as well! Night two will be the big game… We will see who has an unstoppable strategy, all while learning more about our friends and peers. How exciting! We will take you on a delectable adventure of the Kingdom of Sweets, complete with the Gumdrop Mountains and Lollipop Woods. Enjoy a delightful dinner of tossed salad, Italian potatoes, glazed carrots, roast beef, rolls, and yummy dessert bars, catered by the terrific Master Grillers. NEW PROGRAMS & SPECIAL EVENTS RIVER CITY SPINNERS PROGRAM DESCRIPTION: A social group for adults with mild intellectual and/or physical disabilities, who may be socially isolated within our community. Participants must exhibit good social behavior, have no major medical concerns, and display appropriate behavior toward property, themselves, and others. Social activities are scheduled three to four times per month. (Participants requiring medication during the event, must bring only necessary dosage and it must be labeled with medication name, dosage, and directions.) Staff to participant ratio is 1:6 or 1:7. Ratios of 1:1 or 1:2 are not available. Please note the meeting place for each activity. Limited to 12 people per activity unless otherwise specified. REGISTRATION PROCESSING BEGINS: DECEMBER 5 TH , 2014 Friday, January 9th: Guys’ Night at the Boat & Outdoor Show Calling all outdoor enthusiasts! Come with us to the Iowa State Fairgrounds to check out this year’s Boat & Outdoor Show! For the boaters, there will be more than a dozen different brands of boats on display, ranging from pontoons to speedboats to fishing boats. There will be exhibits for the fishers, for the hunters, and for the outfitters. What a great distraction from the harsh winter blues! Saturday, January 17th: The Omaha Durham Museum Durham Museum located in downtown Omaha, Nebraska, opened its doors as a Union Pacific station in January of 1931! The station was one of the busiest in the nation with10,000 passengers utilizing it every day! Now the station has been transformed into the Durham Museum. This remarkable building houses some of Omaha’s most unique treasures and provides a captivating look at the history of the region. Here you can find many exhibits, varying from history to science, and culture to industry. Come and explore this unique museum and all the beauty that it offers! Weight Lifting Were changing locations!! After many years of going to the Riverfront YMCA, we will now be going to the NEW Wellmark YMCA! On Monday and Wednesday afternoons we will work on strength, coordination, flexibility, and cardiovascular endurance. Each session will have three parts: walking on the track for warm-up, lifting weights with machines and free weights, and doing cardio with treadmills, stationary bikes, and ellipticals. This is a fun way to get in shape and stay fit. We will begin Monday, January 5th and continue through Monday, April 20th We will also have a Friday afternoon session for those who wish to train for the Special Olympics Power Lifting competition at the Mid-Winter Games in Iowa City on March14th. Those who choose to participate must register for this activity. Athletes must decide if they want to compete in power lifting or basketball, as they are not able to do both. Athletes must use a properly fitting weight belt. Bocce **Please note changes in start dates, location, and other details** Bocce ball is an ancient game that is gaining popularity. Modern day Bocce ball resembles bowling in that you roll a ball towards a target. The goal is to get the 16 Special Olympics Athletics NEW PROGRAMS & SPECIAL EVENTS ball closest to the target. We will start each practice with a warm up and then focus on teaching skills for playing Bocce. As the season progresses, teams will have the chance to play against each other. Special Olympics Bocce is a team sport consisting of 4 players on a team with a possibility of 4 alternates per team. All teams will compete at a new-this-year regionals qualifying tournament – details to be announced. Those participating in Bocce will not be able to participate in Soccer. Keep in mind the last day to register and turn in S.O. physicals will be January 5th, 2015. New Bocce participants must attend the first practice on January 5th from 5:30-6:30 pm for mandatory assessments. All other participants should attend this practice as well.
