Do CBC, platelets. Check Creatinine if patient over 75 yo. Labs must be WNL. Brilinta (Ticagrelor) – Discontinue for 5 days Coumadin(Warfarin)( Warfilone) - Discontinue and treat when PT( Prothrombin) or INR is normal. If patient has been off Coumadin for 3weeks, labwork is not necessary. The INR must be 1.4 or less. Desirudin (lprivask) – Discontinue for 24 hours and check PTT. PTT should be normal Effient (Prasugrel)- Discontinue for 7 days. Check with prescribing MD first! Eliquis (Apixiban) – Discontinue for 48 hours. No labs required Elmiron – Discontinue 3 days prior to treatment Fragmin-(Dalteparin)- Discontinue for 24 hours prior to treatment Heparin- Discontinue and treat when a PTT ( Activated Partial Thromboplastin Time) is normal Innohep ( Tinzaparin) – Discontinue for 24 hours prior to treatment Integrilin (Eptifibatide)- ( IV med) Discontinue 12 hours prior to treatment. (Consult with cardiologist re: stopping) Jantoven—( Same as Coumadin) Discontinue and treat when PT( Prothrombin) or INR is normal. If patient has been off Coumadin for 3weeks, labwork is not necessary Lovenox ( Enoxiprin) - Discontinue for 12 hours prior to treatment Persantine (Dipyridamole) – Discontinue for 72 hours Plavix ( Clopidogrel)- Discontinue for 5 days prior to treatment Pletol ( Cilostazol) – Discontinue 4 days prior to treatment Pradaxa - Should be discontinued for 5 days. If less than 5 days, draw PTT. The PTT must be within normal range. Ticlid ( Ticlopidine HCL) - Discontinue for 14 days, must do CBC with platelets 1-2 days prior to treatment Tyvaso (Treprostinil, Remodulin) – Discontinue for 24 hours Xarelto (Rivaroxaban) Discontinue for 72 hours. If less than 72 hours draw PTT. Must be WNL.
