Renovação do Auxílio Infantil
Beneficiários: famílias com crianças com
menos de 15 anos ou que completam 15 anos
durante o ano fiscal de 2016 que estão
recebendo o Auxílio Infantil precisam
apresentar o pedido de renovação.
Documentos necessários: apresente o
formulário específico (enviado no início de
junho) devidamente preenchido até 30 de junho
(qui) na prefeitura ou sub-sedes. A renovação
pode ser enviada por correio: são válidas as
correspondências com data de postagem até
30 de junho (qui). É necessário enviar cópia do
Cartão de Seguro de Saúde de todas as
crianças listadas no formulário enviado.
Verifique atentamente documentos e
procedimentos necessários na explicação anexa
ao formulário. Atenção: A falta ou atraso da
renovação implicará na retenção dos benefícios.

Child Allowance Renewal
Application
Renewal application form will be sent to
families receiving Child Allowance as of May
2016 who are currently raising child up to
junior high school age. Civil servants (except
temporary staff) must submit their application
at their working place. Don’
t forget to submit
the Current Status Report Form (renewal
application form) or the Child Allowance will be
suspended.
What to submit: Current Status Report
Form sent in the beginning of June (please
contact the Juvenile and Family Division if you
are eligible but haven’
t received the form yet).
Copy of the health insurance card of the
beneficiary and all the children listed on the
application form. ※Additional documents may
be required. Submission place and
deadline: Juvenile and Family Division, branch
or sub-branch offices, by Thursday, June 30.
