私は、オックスファム・トレイルウォーカー東北2015に参加を希望します。
全イベントを通して、
自己責任で行動すること、
また、本イベントに関連する
全ての事柄に関して、死亡、怪我、財産の損害・損失・破壊が発生した場合、
主催者、
スポンサー、協賛機関に責任を追求しないことを承諾します。
また、主催者に損害を加えた場合、その金額を賠償または返還することを
承諾します。
私は、
(a)オックスファム・トレイルウォーカー東北2015がファンドレイジング
イベントであり、オックスファム・ジャパンのために最低6万円を集めることを、
ここに確認し、承諾します。
(b)オックスファム・トレイルウォーカー東北2015への参加を通して、
直接・間接に得た支援金は、全額をオックスファム・ジャパンに寄付します。
私は、オックスファム・ジャパンが、
プロモーション活動、支援金収集活動に際して、
オックスファム・トレイルウォーカーに関するあらゆる宣伝資料（ウェブサイトを含む）
のために、オックスファム・トレイルウォーカー東北2015とそれに関連する活動に
参加するサポート隊を含めたメンバーの写真、映像、ビデオテープ、
レコード、
またその他の肖像・声の複製を（照会・事前確認・支払いなしに）使用することを、
ここに確認し、承諾します。

I, the undersigned, wish to enter Oxfam Trailwalker Tohoku2015. I confirm and agree that
I take par in this event entirely at my own risk and that I will not hold the organiser or any
sponsor / supporting organisation, responsible for death or any injury or for any damage
to or loss or destruction of property or any other economic loss with this event.
I also agree to indemnify or reimburse the organiser in respect of any additional
expenses or costs incurred by the organiser arising from or in connection with my
participation in this event.
I, the undersigned,
(a) acknowledge that Oxfam Trailwalker Tohoku2015 is a fundraising event.
My teammates and I confirm and agree hereby that we shall raise a minimum of
60,000yen in sponsorship for Oxfam Japan;
(b) undertake that all funds raised directly or indirectly from my particpation in
Oxfam Trailwalker Tohoku2015 will be paid in full to Oxfam Jpan.
I, the undersigned,
hereby acknowledge and agree that Oxfam Japan may (without reference to,
and without the prior approval of, or any payment to, any person) use,
in any publicity material connected with Oxfam Trailwalker (including but not limited
to any of our websites) or in connection with any of our promotional or fundraising
activities, any photograph, film, videotape, record or other reproduction of the image
and / or voice of me, my teammates or members of my support team who take part
in Oxfam Trailwalker Tohoku2015 and is related acitivities.
