久保田豊基金助成金申請書 ★募集要項をよく読んで下さい。 Please carefully read application guidelines. 2015年1月9日（金）までに[郵送とE-mail]で久保田豊基金へ直接提出 January 9 (Fri), 2015 Submit the documents to The Kubota Fund by Mail & E-mail. ①をｐｄｆファイルでE-mailで送付 ①～④を郵送で送付 ※ 両方の方法で書類を提出してください。 提出書類ﾁｪｯｸ/Confirm application documents ① 助成金申請書 Grant Application E-mail ■注意／Notes * 申請書は日本語でも英語でも構いませんが、手書きは禁止します。 You can write in Japanese or English. Application must be typed. Handwritten application will not be accepted. post * E-mailで送る時は、写真と署名は不要です。 Send by E-mail once. No need to put a Photo or your signature. * 郵送で送る時は、写真を貼り、最後のページの日付と署名を直筆で記入して、 ②～④と一緒に送付してください。 Send by Post, together with the other 3 documents. Do paste your Photo, handwrite the date and sign on the last page. ② 在学証明書 Certificate of Enrollment ③ 指導教員からの推薦状 Recommendation from your academic advisor ④ 母国の大学/大学院の成績証明書（最終学年（B4/M2)の成績が記載されているもの） Transcript of Bachelor or Master Course (for the final year) of the college/university/graduate school in your mother country.
