Applying for the Research Grant-in-Aid
from Miyakawa Memorial Research Foundation in the Fiscal Year 2015

Terms for Receiving the Grant
助成金の使用及び研究成果の報告について
１ You are requested to make public the results of studies supported by
the Grant in paper(s) published in medical jounal(s),in which you must
acknowledge the support from Miyakawa Memorial Research Foundation.
研究成果を論文として発表することを原則と致します。
また、発表に際しては助成金支給に関する記述を入れることをお願いいたします。

Within one month after the end of fiscal year 2015 (by April 30, 2016)
you are requested to submit the Following two materials to the office
of Miyakawa Memorial Reseaｒch Foundation.
ⅰ)The scientific report of your studies supported by the grant.
ⅱ)List of itemized and detailed expenses defrayed by the grant along with
the receipts of payment
研究助成金の使用報告
ⅰ）研究終了後、予算年度末までに研究報告書を提出すること。
ⅱ）助成金の使途について報告すること。支払いに対する証明書を添付すること。
（平成２８年４月３０日まで）

You are requested to reimburse the grant in full amount to the foundation
when you infringe the following requisites
ⅰ )You must not make any false declaration or improper maneuvers in
applying for the grant.
ⅱ)You must not spend any parts of the grant to purposes other than
those specified in your application.
研究助成金の返還
次のいずれかに該当するときは、給付した助成金の返還を求めることがあります。
ⅰ）偽りその他不正な手続きにより助成金の給付を受けたことが判明したとき。
ⅱ）助成金をその目的以外に使用したとき。
We wish you to make the most of the grant and accomplish useful scientific
contributions to the medical society in the world.
