U vindt in deze brochure een overzicht van de leveranciers waar voor de PZP vanaf 2014
een contract mee is afgesloten. Per leverancier ziet u welke bloedglucosemeters worden
aangeboden.
Categorie-indeling
U ziet in het overzicht de verschillende leveranciers en de bloedglucosemeters die ze per
1 juli 2014 aanbieden. Deze meters zijn ingedeeld in categorie 1 t/m 4. Deze indeling geeft
aan welke bloedglucosemeter uit het assortiment past bij het aantal momenten dat u
insuline spuit. Zo bent u verzekerd van een meter die goed aansluit op uw persoonlijke
situatie en zorgbehoefte. Mogelijk krijgt u ook advies van uw diabetesverpleegkundige,
huisarts of specialist. Zij brengen uw medische situatie in beeld en kunnen u advies
geven over uw bloedglucosemeter. Vervolgens zoekt de leverancier daar passend
diabetestestmateriaal bij.
De meetresultaten van de beschikbare meters voldoen aan de nieuwste ISO-norm
(15197:2013) als het gaat om nauwkeurigheid.

Behandeling met insuline wordt overwogen.
U bent nagenoeg uitbehandeld met orale bloedsuikervervangende middelen
1Bzwangerschapsdiabetes
U hebt zwangerschapsdiabetes met of zonder gebruik van insuline
2 normaal
U hebt 1 tot 2 maal daags een insuline-
injectie nodig
3A intensief (standaard)
U hebt 3 of meer insuline-injecties per dag nodig
3B intensief (uitzondering)
U hebt 3 of meer insuline-injecties per dag nodig en het is voor u medisch noodzakelijk dat de bloedglucosemeter aan specifieke persoonsgebonden eisen voldoet
4 insuline-pomp
U bent gebruiker van een insulinepomp

afspraken zijn gemaakt in 2014. Alle leveranciers bieden veel keuze in bloedglucosemeters
en diabetestestmateriaal. U kunt zelf kiezen naar welke leverancier u wilt. De contactgegevens van alle leveranciers staan in deze brochure.
