Important Information Regarding G-Codes!!
What’s a G-Code?
Beginning July 1, 2013, the use of G-codes and their modifiers is mandatory by Medicare for Part
B patients. If a Medicare Part B claim is submitted to Medicare that does not have the G-code
and modifier after July 1, 2013, the claim will be denied. The G-Codes and modifiers must be
present in both the medical record and the claim. In particular, the medical record is supposed
to show what evaluation tools were used by the therapist that led to the values submitted on
the claim form.
What is a G-code modifier?
G-codes must include a modifier that objectively measures the percent of disability at
evaluation, goal, and discharge.
How can I learn more about G-codes and objective tests and measures?
