REDCap instructions for EuroSIDA Follow-up (FU 41 Winter 2014/2015)
ACCESS
Enter the address: https://www.chip-crf.info/redcap/index.php?action=myprojects into your browser and
log in with the username and password that has been sent to you. The first time you log on you will be
asked to change the password and set up a password recovery question.
Please note that access is personal, and if more staff at your site need access, please contact
[email protected]
MY PROJECTS
You should have access to one or more of these 5 projects:
EuroSIDA Follow-up (FU 41 Winter 2014/2015): All follow-up forms should be completed electronically in
REDCap. The follow-up form contains several sections, which should all be reviewed and/or completed.
EuroSIDA Enrolment: All enrolment forms should be completed electronically in REDCap. The Enrolment
form contains several sections, which should all be completed.
EuroSIDA HCV Treatment Form: Please complete the electronic REDCap form for patients who:

are newly enrolled into EuroSIDA, AND are receiving HCV treatment at the time of enrolment.
are already enrolled in EuroSIDA, AND have started HCV treatment since last follow up.
HCV Treatment Adverse Event Form: Please fill out the electronic REDCap form if a patient has stopped
HCV treatment earlier than planned due to toxicity/intolerance.
TEST EuroSIDA on RedCAP TEST: test forms in which you can “play around” and get familiar with redcap.

EDIT RECORD
The EuroSIDA Follow-up (FU 41 Winter 2014/2015) form has preloaded patients records with the
information you have previously provided in enrolment and/or previous follow-up forms. Preloaded data is
listed in grey fields.
To edit a record:
1. Click one of the projects (i.e. EuroSIDA Follow-up (FU 41 Winter 2014/2015), EuroSIDA HCV Treatment
Form or Adverse Event Form). The project setup page will appear.
2A. Click Record Status Dashbord on the left hand side under Data Collection
and then click the status icon for the patient and section you wish to edit.

2B. Or if you know the patient ID, then click Add/Edit Records on the left hand side under Data Collection
Type the EuroSIDA Patient Identification code [centre code (3 digits) followed by patient code (4 digits) i.e.
1501515] and press ENTER on your keyboard.
3. The EuroSIDA Follow-up (FU 41 Winter 2014/2015) contains a form for each section. You can navigate
between the different sections using the links on the left hand side:

or the Record Status Dashbord:
The coloured icon in front of the form denotes the status, i.e.:
- Green (complete) = The form/section is complete.
- Yellow (unverified) = The form/section has been edited, but is incomplete.
- Red (incomplete) = The form/section contains preloaded data and has not been reviewed and/or the
form/section has been saved, but data is incomplete and will be completed/reviewed later.
- Grey (incomplete) = No data has been entered/saved.
4. Fill out the forms/sections. Data should be entered in the yellow fields. Previously reported data is
displayed in dark gray fields. Please do not edit previously reported data, unless you find an error. In some
sections, e.g. section D, E1 and E2, the yellow fields are located amongst the gray fields.

To remove an answer, click Reset.
To report antiretroviral treatment that started, stopped and restarted the same drug between last and
current visit:
Comma vs. full stop
You have to use full stop instead of comma. Otherwise you will receive this error:

5. Select status at the bottom of the form/section.
Incomplete: The form/section contains incomplete data and will be completed later.
Unverified: The form/section contains incomplete data and will be completed later. Please use this status if
you are interrupted and need to come back to it later. Using this status will make it easy to find the form
again via the Record Status Dashbord .
Complete: The form/section contains all available data.
6. Click Save Record, Save and Continue or Save and go to Next Form.
Save Record: Saves and exits record.
Save and Continue: Saves and lets you continue entering data (same record).
Save and go to Next Form: Saves form and opens the next form for the current record
ADD RECORD
The EuroSIDA enrolment form, EuroSIDA HCV treatment form and HCV Treatment Adverse Event Form do
not have preloaded patient records - you need to add a new record.
To add a new record:
1. Click one of the projects (i.e. Enrolment Form, Treatment Form or Adverse Event Form). The project
setup page will appear.
2. Click Add/Edit Records on the left hand side under Data Collection.
3. Type the EuroSIDA Patient Identification code [centre code (3 digits) followed by patient code (4 digits)
i.e. 1501515] and press ENTER.
