Read through the vacation care brochure 2. Complete a 2015 Enrolment Form for Epping North OSHCP: • • HOURSOFOPERATION 7:00am – 6:30pm. All children must be at the Centre by 8:30am on excursion days; this is to allow staff to give instructions to children. For most excursions we leave at 9am sharp. COST • Cost is per child as stated on the program • Child Care Benefit will be calculated at time of enrolment for eligible families • Prices for each day will increase by $5 for any bookings made after 9am Friday 12th December 2014 • Late Pick Up Fee: $10.00 for first 5 minutes, $20.00 per 5 minutes thereafter • Non notification fee of $15 will be charged for failure to notify the Centre of your child’s absence WHATTOBRING • Yellow vacation care hat • Sensible secure shoes - no thongs • Refillable drink bottle • Sun smart clothes – shoulders should be covered • Wet weather gear/safety gear eg helmet • Morning tea, afternoon tea and lunch on excursion days • Each child must bring their own bag on excursions. • Spare clothes in case of an accident ArrivalsandDepartures It is a legal requirement that parents sign their child in and out each day. Children will only be permitted to leave the Centre when accompanied by an adult nominated on your enrolment form. Meals Breakfast is provided each day until 8:00am. We provide morning tea, lunch and afternoon tea on the days we stay at the Centre. Parents must supply all meals and drinks on excursions (unless otherwise stated on the program). We suggest that the food you provide your child is healthy and nutritious. Re-enrolling families: complete your 2015 enrolment form online using your parent log in ID and password New families: o Contact the Centre to access the current password o Log onto the Centre’s website o Click “WEBSERVER” on the home page o Enter the password o Enter your details o You will then receive an email with your log in ID and password which will enable you to log on through the server to complete your enrolment form and make bookings. o To complete your enrolment, send to the Centre your child’s up to date ACIR immunisation statement and any health plans. 3. How to make a booking: o Go to our website www.eppingnorthoshc.com.au o Click “WEBSERVER” on the home page o Enter your USERNAME and PASSWORD o Click “PARENTS MENU” o Click “VACATION CARE BOOKINGS” o Follow the instructions in the “HELP” button at the bottom of the screen to make your bookings 4. Make payment to the Centre, unless you are on direct debit 5. If required, send your permission note and hat order form to the Centre. th • Bookings are required by 9am Friday 12 December 2014and need to be made online through the webserver on the Centre’s website. • Bookings made after this date will be accepted if there are places available. • Daily prices increase by $5 for any bookings th made after 9am Friday 12 December 2014. th • Payments need to be made by 9am Friday 12 December 2014. Those on direct debit will be invoiced and charged at the end of the holiday period. PAYMENTS Your options for payment are internet transfer or direct debit. This will be charged at the end of the holiday period. Contact the Centre for a direct debit application form. Refunds There are no refunds, credits or transfers after the closing date unless a medical certificate is supplied in your child’s name for the day/s missed. You must provide the certificate to the Centre no later than the Friday of the week of absence otherwise no refund can be given. 24 hour notice is required for absent days that occur on an excursion day. ANAPHYLAXIS,Allergies,MEDICATIONS If you child is anaphylactic or has an allergy which requires your child to carry medication, a full kit of your child’s medication must be provided to the Centre on or before your child’s first day of vacation care. This kit will need to remain at the Centre until your child’s last day of care. Medication can only be administered by staff with written permission from the parent and that has been prescribed by a doctor. Over the counter medication will only be administered if accompanied by a letter from the doctor stating the child’s name, the dosage and time required. NutFreeCentre ENOSHCP is a nut-free Centre. We have many children who attend the Centre that are anaphylactic to nuts. We would appreciate you do not sending your child with any nut products including Nutella spread. Sunprotection The Centre follows the guidelines set by the Cancer Council. Yellow vacation care hats are compulsory at ENOSHCP. The hats are either legionnaire or bucket style where face, ears and neck are covered. Please also ensure they wear closed-in shoes and suitable sun-safe clothing, i.e. t-shirts that cover their back and shoulders. The Centre provides sun cream which children will be expected to reapply throughout the day. If your child is sensitive to sun cream please ensure you provide your own sun cream in your child’s bag. Yellowvacationcarehats All children will need to wear a yellow vacation hat to ENOSHCP. These are available from the Centre at a cost of $7 each. These hats are all sun safe and greatly assist staff in identifying the children when out on excursions. If your child arrives without their yellow hat, your child will be given a new hat and your account will be charged a $7 yellow hat fee. Excursions Places are limited. All excursions are compulsory if your child is booked in on that day. Your child must be here by 8.30am. This is to enable staff to give adequate instructions to the children and prepare them for the day. Buses leave 9am sharp, if your child is not here we will assume your child is absent for the day and the bus will depart at 9am. The Centre will not make phone calls to check if you are still attending. Children are not to bring money excursions. They must wear the yellow hats that are available for purchase from the Centre. All children must have their own bag. On some excursions siblings will be split, eg due to age, height or other restrictions. It is essential that siblings so not share bags in case this scenario occurs. Lostproperty Please clearly label all items your child brings to vacation care. Anything left at the Centre 4 weeks after the conclusion of a holiday period will be donated to charity. FURTHERINFORMATION The Centre reserves the right to make changes to the program without notice, including cancellation of programs due to insufficient numbers or extreme weather conditions. Program leaders reserve the right to cancel a participants enrolment should their behaviour be deemed unacceptable or is placing other children or staff at risk. Participants are responsible for their own belongings. The Centre will not be held responsible for the loss of property. PRIVACYSTATEMENT ENOSHCP collects personal and health-related information for the purpose of registering your child for the upcoming Vacation Care Program. We are committed to protecting confidentiality; all information is stored securely. If you chose not to provide this information, we may not be able to finalise registration.
