Guidance Notes for completing application form
Declaration
We require youmake a declaration by signing the completed form.
Parental Consent Form
We require all participants to seek parental consent to participate in the Face of Teens Pageant. This part of
the form needs to be completed by your parents or guardian. They must give their consent by signing the
completed form.
Rules/Guidelines:
Delegate and Parent acknowledges that being a participant in the Face of Teens Pageant includes abiding by
the rules of conduct and the participation rules set forth in this agreement. If at any time a participant does not
abide by all policies, rules, and regulations set forth by the Face of Teens Pageant, immediate disqualification
may result. The parties agree to the following:
1. Age:
 Must be a Ghanaian Resident.
 Must meet the age requirements as of the date of the pageant. Contestant must be at least 14 years of age
and no older than 19 years of age by January 1st of the competition year.Must provide an offical copy of Birth
Certificate.
 No unauthorisedphotography allowed on or infront of the stage during the event. Pageant photographer will
be on hand to take all stage shots and posed shots of your child during and after the competition. You can take
pictures from your seat to allow all in attendance to witness the whole ceremony without interruption.
2. Code of Conduct:
 Any attempt to communicate with the judges before or during the pageant will result in immediate
disqualification.
 Good sportsmanship is expected at all times. Disruptive behaviour before, during, or after the pageant will
result in disqualification.
3. Selection of Judges:
 Judges decisions are final.
4. Participants Responsibility:
 Formal Wear as per competition guidelines MUST be worn for crowning. No exceptions.
 No refunds will be given.
 Entry fee must be paid by Cash orBankers Draft.
5. Liability:
 Participants or Parents will not hold the organization, its directors, judges and staff members or the pageant
location liable for any illness, injury or loss incurred before, during, or after the pageant.
 Pageant reserves the right to change the format of the event at any time without consultation.
 Pageant reserves the right to break all title holder ties through questions and answers or voting by Judges
with consultation from Pageant Directors.

 Pageant reserves the right to any and all names and photos for purposes of publicity, advertising, and
promotion in print, web and video.
Fees:
Face of Teens Application Fees: Gh₵ 30.00. This is what you pay to participate in the audition.
Face of Teens official sponsorship Entry Fee:
GH₵400.00
This is what you pay to participate in the official pageant when selected. The office will assist you in your fund
raising. Participants will camp for 7 days before the pageant to undergo series of workshops, events, projects,
programmes and rehearsals.
Photo Guidelines:
You must submit two colored pictures (one full with legs exposed and one potrate) but the size must be a least
3½ x 5”. It is highly suggested that only actual photos be submitted for best quality. You may submit them
electronically.
Please include the contestants name when sending the photo. If you are sending the photo electronically
please include the information in the email.
We encourage everyone to obtain sponsors to avoid spending personal money to participate.
