Division of Administration:
1. Resolution to approve certain appropriation transfers (per the attached list).
2. Resolution to approve a renewal agreement with Diane Zucker for consulting
services, compliance audits and training sessions on documentation and coding for
Medicaid reimbursement in an amount not to exceed $7,000 for the period January 1,
2015 through December 31, 2015.
3. Resolution to approve a renewal agreement with Montgomery County Microfilming
Board for microfilming services in an amount not to exceed $10,000 for the period
January 1, 2015 through December 31, 2015.

4. Resolution to approve a renewal agreement with the Board of County Commissioners
of Montgomery County, Ohio, authorizing the performance of certain custodial
services including the emptying of all office trash receptacles in the Reibold Building.
The initial term of this agreement will be from January 1, 2015 through December 31,
2015 with four consecutive one year renewals. For the period January 1, 2015 through
December 31, 2015 the total not to exceed amount is $12,000.
5. Resolution to approve a two year renewal lease agreement with Higher Ground
United Methodist Church to house the Community Health Outreach staff in an
amount of $1,050 quarterly for a total not to exceed amount of $4,200 for the period
January 1, 2015 through December 31, 2015.
6. Resolution to approve an amendment to the 2014 agreement with Cirrus Concept
Consulting, Inc. (Resolution #13-376, dated December 4, 2013) increasing the not to
exceed amount of this agreement to $149,000 for the period January 1, 2014 through
December 31, 2014.
7. Resolution to approve the second year of a two year grant providing limited funding
to the Community Health Centers of Greater Dayton for dental services to be
provided at the East Dayton Health Center in an amount not to exceed $75,000 for the
period January 1, 2015 through December 31, 2015.
8. Resolution to approve an agreement to provide limited funding to the Community
Health Centers of Greater Dayton for the provision of primary health care services to
low income and medically needy families in Montgomery County in an amount not to
exceed $500,000 for the period January 7, 2015 through March 30, 2016.
9. Resolution to authorize Public Health – Dayton & Montgomery County to participate
in the State of Ohio Cooperative Purchasing Program under the conditions and
circumstances set forth in the Resolution.
10. Resolution to approve a policy titled Equal Employment Opportunity (EEO) for
inclusion in the Personnel Policy Manual.
11. Resolution to approve a policy titled Health Care Benefits for inclusion in the
Personnel Policy Manual.
Division of Disease Prevention:
12. Resolution to accept the Notice of Award for the Sexually Transmitted Disease
Control Grant in the amount of $62,899 for the period January 1, 2015 through December
31, 2015.
13. Resolution to accept the Notice of Award for the HIV Prevention Grant in the amount
of $427,317 for the period January 1, 2015 through December 31, 2015.

14. Resolution to accept the Notice of Award for the Immunization Action Plan Grant in
the amount of $181,397 for the period January 1, 2015 through December 31, 2015.
15. Resolution to accept the Notice of Award for the Tuberculosis Grant in the amount of
$34,902 for the period January 1, 2015 through December 31, 2015.
16. Resolution to approve an amendment to the intergovernmental agreement with the
Montgomery County Family and Children First Council to provide services for the
Montgomery County Fetal Alcohol Spectrum Disorders (FASD) Community
Capacity Building Project in an amount not to exceed $44,543.76 for the period January
1, 2015 through December 31, 2015.
17. Resolution to approve an agreement with Lion + Panda for a marketing campaign
and branding development for the Fetal Alcohol Spectrum Disorder (FASD)
Coalition in an amount not to exceed $15,322 for the period January 1, 2015 through July
31, 2015.
18. Resolution to approve an agreement with the AIDS Resource Center Ohio to verify
follow-up for HIV positive individuals referred for services who reside in Region 7
(Butler, Darke, Miami, Montgomery, Preble and Warren Counties) as per the
Linkage to Care program for the HIV Prevention Grant. The term of this agreement
shall begin on January 7, 2015 and extend through December 31, 2015. There is no
monetary obligation by either party associated with this agreement.
19. Resolution to approve an agreement with the AIDS Resource Center Ohio to provide
Counseling, Testing and Referral (CTR) focusing on Men Having Sex with Men in
Butler and Warren counties; Healthy Relationships focusing on HIV Positives; and
Mpowerment focusing on All Men having Sex with Men in an amount not to exceed
$49,000 for the period January 7, 2015 through December 31, 2015.
20. Resolution to approve an agreement with Caracole, Inc. to verify follow-up for HIV
positive individuals referred for services who reside in Region 7 (Butler, Darke,
Miami, Montgomery, Preble and Warren Counties) as per the Linkage to Care
program for the HIV Prevention Grant. The term of this agreement shall begin on
January 7, 2015 and extend through December 31, 2015. There is no monetary obligation
by either party associated with this agreement.
21. Resolution to accept an agreement with Darke County General Health District to
provide activities supporting the Ohio HIV Prevention Statewide Initiative within
Darke County, Ohio in an amount not to exceed $1,300 for the period January 7, 2015
through December 31, 2015.
22. Resolution to accept an agreement with Miami County Public Health to provide
activities supporting the Ohio HIV Prevention Statewide Initiative within Miami
County, Ohio in an amount not to exceed $1,300 for the period January 7, 2015 through
December 31, 2015.

23. Resolution to accept an agreement with Middletown City Health Department to
provide activities supporting the Ohio HIV Prevention Statewide Initiative within
Butler County, Ohio in an amount not to exceed $10,000 for the period January 7, 2015
through December 31, 2015.
24. Resolution to accept an agreement with Preble County General Health District to
provide activities supporting the Ohio HIV Prevention Statewide Initiative within
Preble County, Ohio in an amount not to exceed $1,300 for the period January 7, 2015
through December 31, 2015.
25. Resolution to accept an agreement with Warren County Combined Health District to
provide activities supporting the Ohio HIV Prevention Statewide Initiative within
Warren County, Ohio in an amount not to exceed $10,000 for the period January 7, 2015
through December 31, 2015.
26. Resolution to accept an agreement with Wright State University, Integrated
Continuum-of-Care Services (ICS) Project in an amount not to exceed $5,810 for the
period September 30, 2014 through September 29, 2015.
27. Resolution to approve Amendment #6 to the Subagreement with Wright State
University, Fifty Plus Prevention Project F3P #5U79SP016434-05 (Resolution #13-290
dated October 2, 2013) increasing funding by $20,000 for a new total amount of
$105,000 for the period September 30, 2014 through September 29, 2015.
Division of Environmental Health:
28. Third and Final Reading. Resolution to approve the proposed 2015 Environmental
Health Division license fee changes in the Household Sewage Treatment System
(HSTS) program per new ODH Rules that become effective January 1, 2015 and per
prescribed cost methodology. Such fees shall be effective on the earliest date allowed
by law.
Office of the Health Commissioner:
29. Resolution to accept the Notice of Award from the Ohio Department of Health for a
Preventing Obesity, Diabetes and Heart Disease and Stroke in Ohio Communities
grant in the amount of $400,000 for the period February 1, 2015 through September 29,
2015.
30. Resolution to accept the Notice of Award from the Ohio Department of Health for
continuation of the Creating Healthy Communities Program grant in the amount of
$125,000 for the period January 1, 2015 through December 31, 2015.
31. Resolution to approve an agreement with the Montgomery County Family and
Children First Council to support the Second Step Project on violence prevention in
the amount of $16,280 for the period January 7, 2015 through December 31, 2015.

32. Resolution to accept a donation from CareSource in the amount of $1,000 for the
Dayton Council on Health Equity in support of the 2015 Minority Health Month
Kick-off Celebration and community events.
33. Resolution to accept a donation from March of Dimes in the amount of $1,200 in
support of the Infant Mortality Coalition’s community outreach to target
populations.
