Facility Move-In Checklist
Before selecting a facility, consult with us to determine which facility may best
suit the Client’s medical needs, offer quality of life, and work within your budget
now and for the long term. Ask about the potential for financial aid/government
programs.
• Change mailing address or re-route to responsible party.
• Update Power of Attorney, Living Will, Health Care Surrogate and other legal
documents. Provide copies to doctors, facility, banks, income sources and others.
• Secure valuables.
Ask Us About The 4 C’s
of Facility Selection
• Make arrangements for pets.
• Disconnect utilities & transfer phone service and cable/internet to the new
facility.
• Return rented equipment cable TV box, medical equipment and emergency
alerts.
• Determine if insurance / HMO changes must be made.
• Select physicians who can visit the facility or arrange transport to outside
doctors.
• Notify Social Security, pensions, and other income sources of your new location.
• Arrange auto bill pay for applicable bills.
• Arrange for sale of home, car, boat, and other un-needed personal affects.
• Coordinate the first day’s medication with facility. You need to bring a three day
supply.
• Pack 7 – 10 days worth of appropriate clothing and label everything in a discreet
location. Label shoes, electric razors, toiletries, etc.
• Bring no expensive or sentimental items or clothing.

his guide is designed to assist in understanding the primary programs
available to those who may need to reside long term, in a nursing home,
assisted living or in their own home with home care services.
Information and Referral
Many Elders and their Families are capable of making their own
arrangements and decisions. However, in order to do so, they need
accurate information, education and direction. The experienced staff
at our parent company, V.I.P. Care Management, Inc. are Elder Care
professionals with vast knowledge of the community resources.
Elders and their Families often find that the information we provide
saves them time, money and frustration.
V.I.P. Care Managers assess the Client’s needs, develop a Care Plan and
coordinate all services and benefits available from professionals, private
providers, non-profit providers, government agencies and health care
facilities. We often work together with other specialists in the fields of
Law, Financial Planning, Insurance, Geriatric Medicine, Home Health
and others.

Social Security Disability
V.I.P. Benefit Services of America specializes in assisting with applications for
disability benefits for persons under 65. A person is Disabled, according to Social
Security rules, when their illnesses, injuries or conditions prevent them from
doing past work or adjusting to other work.
SOCIAL SECURITY USES A FIVE STEP PROCESS
TO DETERMINE ELIGIBILITY:
IS APPLICANT WORKING?
If the Applicant is working and earnings average more than $1,090.00 per month
($1,820.00 if blind), the Applicant generally cannot be considered Disabled.
IS APPLICANT’S CONDITION “SEVERE”?
Applicant’s condition must interfere with basic work-related activities for
the claim to be considered.
IS APPLICANT’S CONDITION FOUND IN THE LIST
OF DISABLING CONDITIONS?
For each of the major body systems, there is a list of medical conditions
that are so severe, they automatically classify an Applicant as Disabled. If the
condition is not on the list, Social Security must decide if it is as severe as a
medical condition that is on the list. If it is, it may be decided that the Applicant
is Disabled.
IS APPLICANT ABLE TO PERFORM PREVIOUS WORK?
If the condition is severe, but not as severe as a medical condition on the list, then
Social Security must determine if the condition interferes with the Applicant’s
ability to do their previous work.
CAN APPLICANT DO ANY OTHER TYPE OF WORK?
If the Applicant cannot do the work they did in the past, Social Security
considers the medical conditions, age, education, past work experience and any
transferable skills. If the Applicant cannot adjust to other work, the claim will
likely be approved.

preparing and submitting Social Security Disability claims quickly and
efficiently in a difficult and complicated government system. Complex cases
may be referred to an attorney.

Nursing Home Medicaid
PAYS FOR LONG TERM CARE
We specialize in helping our clients apply for all the benefits available
to them. Our clients and their nursing homes appreciate our
compassionate, yet efficient, services and the savings we accomplish for
them.
MEDICAID ICP
The Institutional Care Program (ICP) may be available even if the
Applicant owns property and has assets. The program eligibility rules
are complex. We help Elders and their Families overcome the hurdles
of Medicaid eligibility while the Elder receives the most appropriate
medical care.
Medicaid ICP will cover the majority of the COST of living in a
Nursing Home. More information is available at www. my florida.com
BASIC MEDICAID ICP ELIGIBILITY REQUIREMENTS:
AGE
Applicant must be over 65 or Disabled.
V.I.P. Benefit Services of America can help those under 65 apply
for Social Security Disability benefits.
MEDICAL NEED
Applicant must need assistance with Activities of Daily Living.
Specific medical forms are required to demonstrate the need for
nursing home Level of Care.
CITIZENSHIP
Applicant must be a U.S. citizen or resident alien and a Florida
resident.
Proper citizenship documents will be required for application
submission.

INCOME
The Applicant’s gross income may not exceed $2,199.00 per month.
If the Applicant is married and the spouse will remain at home, there
is no income limit for the spouse. If the spouse has an income
below $1,966.00, the spouse may be able to keep a portion of the
Applicant’s income. Otherwise, all or part of the Applicant’s
income may be owed to the nursing home each month.
According to the Department of Children and Families website and
policy manual, an Applicant whose gross income exceeds $2,199.00
per month may still receive Medicaid Benefits as long as they submit
a properly executed and funded Qualified Income Trust. This a legal
document written by an attorney.
ASSETS
A single Applicant’s countable assets may not exceed $2,000.00 and
married Applicant’s assets may not exceed $119,220.00.
V.I.P. Benefit Services of America wants every Applicant to know that
strategies exist which may help to preserve the assets of the Applicant.
A complete list of marital assets must be provided, and the Applicant
should consult with a licensed attorney who will be needed to draft
documents and explain all the Applicant’s options including
asset protection. We will gladly refer clients to a qualified attorney
if they do not have one.
RESPONSIBLE PARTY
The Applicant must be mentally capable of managing their medical
and financial affairs or a legally designated spouse, family member or
other state approved Responsible Party should be selected.
V.I.P. Benefit Services of America recommends that each Applicant
designate a Durable Power of Attorney and a Health Surrogate. This
is accomplished with legal documents which should be written by an
attorney licensed in Florida.

Veteran Benefits
VETERAN AID AND ATTENDANCE PROGRAM
Non-service connected pensions for wartime Veterans and their Widows are a
well kept secret. The Pension with Aid and Attendance is designed to financially
assist aging, low asset and low income homebound Veterans and their unremarried Widows who require home health care givers on a continuous basis
OR who reside in Assisted Living Facilities AND meet all the eligibility criteria.
THE BASIC QUALIFICATIONS INCLUDE:
WARTIME SERVICE WITH HONORABLE DISCHARGE
Applicant must provide proof of at least one day of active duty
during VA designated wartime.

MEDICAID CERTIFIED FACILITY
The Applicant, spouse, family or responsible party must choose a
Skilled Nursing Facility (Nursing Home) that has been Medicaid
Certified. Fortunately, some of the best facilities in Florida hold this
certification.
Our Care Managers at V.I.P. Care Management will be happy to assist
applicants and families in selecting a facility which meets their needs.

A VA form DD214 is required.
ASSETS
For single Applicants, total assets must be less than $50,000.00.
Married Applicants must have less than $80,000.00 in assets.
The amount does not include home equity and one automobile.
A complete list of marital assets must be provided.
INCOME
Veteran Applicant’s monthly income must be less than $1,788.00 if single,
and less than $2,120.00 if married. A widow’s income must be less than
$1,149.00.
Qualifying medical expenses may be deducted from current gross
income to help meet income eligibility requirements.
MEDICAL ELIGIBILITY
Applicant must be homebound and need the assistance of paid
care givers to meet their daily care needs.

ASSISTED LIVING
Those in need of an Assisted Living Facility may access the Medicaid
Long Term Care Managed Care Program which is designed to help pay
for a portion of their care expenses.
Please note: Different from Nursing Home Medicaid ICP, this program
only pays for a portion of the monthly cost of care and delivers limited
services.
V.I.P. Benefit Services of America is able to assist the Applicant and
their family apply for this program and assist with facility placement
and solutions for subsequent care.
HOME CARE AND SERVICES
Medicaid Long Term Care Managed Care Program may help an
Applicant remain in their own home and receive care from a home health
aide that is provided by an approved home care company. Many other
services such as incontinent supplies and medical alert devices may also
be provided through this program.
V.I.P. Benefit Services of America is able to assist the Applicant and
their family to apply for this program and coordinate home health care
services. Our care managers are able to monitor all in home and facility
based care, under our V.I.P. Care Management division.
* In cases of denial, the facility may charge the Applicant the
private pay rate for each month that Medicaid has been denied.
One mistake on a Medicaid application may be very costly.

Facility Move-In Checklist
Before selecting a facility, consult with us to determine which facility may best
suit the Client’s medical needs, offer quality of life, and work within your budget
now and for the long term. Ask about the potential for financial aid/government
programs.
• Change mailing address or re-route to responsible party.
• Update Power of Attorney, Living Will, Health Care Surrogate and other legal
documents. Provide copies to doctors, facility, banks, income sources and others.
• Secure valuables.
Ask Us About The 4 C’s
of Facility Selection
• Make arrangements for pets.
• Disconnect utilities & transfer phone service and cable/internet to the new
facility.
• Return rented equipment cable TV box, medical equipment and emergency
alerts.
• Determine if insurance / HMO changes must be made.
• Select physicians who can visit the facility or arrange transport to outside
doctors.
• Notify Social Security, pensions, and other income sources of your new location.
• Arrange auto bill pay for applicable bills.
• Arrange for sale of home, car, boat, and other un-needed personal affects.
• Coordinate the first day’s medication with facility. You need to bring a three day
supply.
• Pack 7 – 10 days worth of appropriate clothing and label everything in a discreet
location. Label shoes, electric razors, toiletries, etc.
• Bring no expensive or sentimental items or clothing.
V.I.P. Care Management’s Care Managers are available to visit your loved one in
the facility and report to you their progress on an ongoing or short term basis. We
are the eyes and ears of our families.
