Aviation Brief description Consistent with Our BHP Billiton Charter values, we set standards and minimum mandatory controls for the management and minimisation of aviation risk to our workforce in the conduct of their required duties. Aviation management procedure Aviation operations contracted by, or operated exclusively on behalf of, BHP Billiton must be governed by documented aviation management procedures to provide minimum aviation safety expectations. Include all Appendix 1 requirements in an aviation management procedure, obtain authorisation and review it annually for completeness and accuracy. Obtain authorisation for variations to the Flight Safety Foundation Basic Aviation Risk Standard. Review all technical schedules to aviation contracts or aircraft purchases where the total forecast spend is greater than US$250,000 with Senior Manager Aviation Group Health, Safety and Environment (HSE) prior to the issue of the tender. 2. Travel on third-party contracted aircraft Charter aircraft contracted by third-party entities and used by employees must be approved prior to use to meet minimum aviation technical requirements. Obtain authorisation for the use of third-party contracted aircraft. 3. Passenger-carrying flights BHP Billiton contracted aircraft carrying passengers must be fitted with dual controls and be operated by two qualified pilots to provide additional safety assurance. 4. Aviation critical controls Aviation material risks must have a common set of critical controls to provide a minimum control framework for aviation risk management across the BHP Billiton Group. Implement the following critical controls for all aviation material risks: Personnel: pilots meet all licence and experience requirements and are fit for work. Aircraft: appropriate for the activity and suitably equipped. Fuel: acceptable quality and quantity for the activity. Weather monitoring and forecasting: appropriate for the activity and operating environment. Infrastructure: acceptable design and operating conditions for take-off and landing. Aviation management procedure requirements Requirement Designate the individual with single point of accountability for aviation operations Designate the individual accountable for providing specialist aviation input and communication of issues associated with aviation operations Aviation activity description, planning and approvals Use of approved aircraft operators and management of BHP Billitonowned airfields and helidecks Written agreement with aircraft operator Aircraft operator compliance with technical standards of BHP Billiton Simulator training Journey management plans. Risk assessment Explanation Nominate an internal BHP Billiton representative. Name and contact details of an authorised competent aviation specialist. Information about how advice from the competent aviation specialist will be incorporated into aviation operations and communicated with Group HSEC. Include frequency and scope of formal on-site operational reviews by an aviation specialist. Include frequency of formal inspections by an aviation specialist of airstrips and helipads owned or operated by BHP Billiton. Describe the scope of aviation activities being undertaken and covered by the aviation management procedure. Detail the processes to be followed including the required endorsements and approvals prior to aviation operations. Mandate that only authorised aircraft operators may be used. Meet all requirements of the Flight Safety Foundation Basic Aviation Risk Standard. Must be in place for all contracted aviation activities and the technical schedule reviewed by Senior Manager Aviation Group HSE prior to the issue of the tender for contracts or aircraft purchases where the total forecast spend is greater than US$250,000. Aircraft operator documents, procedures and verification processes to ensure personnel involved in scheduling or flying BHP Billiton chartered aircraft know and agree to comply with all requirements of the Basic Aviation Risk Standard. The requirements for when pilots attend simulator training for all long-term contracts to be reviewed with designated aviation specialist. Must be prepared to cover all intended flights and include controls and mitigations for identified journey risks. Performed by the aircraft operator prior to commencement of aviation operations. This must: be reviewed by the competent aviation specialist; BHP Billiton, 19 December 2014 incorporate all documented mitigations from the risk assessment into the journey management plan.
