Applicants are required to read these instructions carefully before filling in the applications forms correctly filled or incomplete application forms will not be processed for admission. 1.0 APPLICATION FEE Submission of duly filled application forms should be accompanied with payment of non refundable application fee. For Tanzanians, the application fee is Tshs. 20,000/= for non Tanzanian, the application fee is USD 30. Remember to use the applicant’s name and to attach the original pay in slip of the application fee to your application forms. 2.0 ADMISSION REQUIREMENTS FOR CERTIFICATE/DIPLOMA IN NURSING The selection criteria will be adhered according to standard set by the ministry of Health and Social Welfare. 1. The following are the criteria to be adhered to:I. Form four or form six leaver male or female aged 16 years and above, II. An applicant with at least 3 pass in Biology, Chemistry, and Physics/Mathematics in CSEE for certificate and at least 2 credits and one pass in the above mentioned subjects for Diploma, III. Physical fitness: physically and mentally fit without any obvious or serious deformity, IV. Pass an entry exam prepared by the school. 2 3.0 SUBMISSION OF APPLICATION FORMS Duly filled application forms and its attached documents should be delivered to the school in the administration office.
