UNIVERSITY HEALTH SERVICE (UHS): CRITERIA FOR THE SUPPLY OF MEDICAL
LABORATORY CONSUMABLES AND REAGENTS
Kindly note:
1. Input the price in front of each item and a dash in front of the unquoted
ones;
2. Adhere strictly to the stated brand;
3. Receipt of yearly renewal as a Supplier to the University;
4. Items supplied must have at least one year expiry dates;
5. Supplies must be delivered to the Medical Laboratory Unit before
12.00noon, Monday to Friday. Supplies brought in after the stated day and
time will not be received;
6. Invoices and Delivery Notes must accompany the supplies in duplicates;
7. Any item or product that falls short of the required standard or quality will
be rejected; and
8. Quotations must be accompanied with:
i.
Evidence of Registration with Corporate Affairs Commission (CAC)
ii.
Current Practising License as a Medical Supplier
iii.
Evidence of Tax Payment/ Clearance
All quotations are to be submitted within 30 days.
