This agreement (“Agreement”) shall govern the terms upon which Bed Bug Training Aids, LLC
(the “Company”) will provide live bed bugs to the undersigned buyer (“Buyer” or “you”) at the property address listed
above (“Property”).
Terms of Service:
Bed Bug Training Aids, LLC will provide the buyer stated above with live bed bugs for purposes of training and or
research in the field of bed bug scent detection. Buyer will not hold Bed Bug Training Aids, LLC, or its officers, owners,
employee’s or affiliates responsible for damages, accidental infestation or unlawful use of the bed bugs, or materials in
which they received. Customer is fully responsible for caring for and maintaining the insects on their own and by signing
below releases Bed Bug Training Aids, LLC from all responsibility. This contract will govern all future purchases of bed
bugs from Bed Bug Training Aids, LLC moving forward. Bed Bug Training Aids, LLC shall not be held responsible for bugs
that have died in travel. For this reason Bed Bug Training Aids, LLC recommends overnight shipping but the decision is of
the buyer. In most cases Bed Bug Training Aids, LLC will include extra bugs to accommodate for any losses.
The Buyer acknowledges that prior to signing this Agreement; it has read this Agreement including the Addendum of
Additional Terms and/or Conditions.
IN WHITNESS WHEREOF, the parties have executed this Agreement as of the date set forth below.
