What is a HRA?
 HRA is just another way of paying for your benefits.
 BlueLinx sets up your account by putting money aside that you can use to pay for
medical and pharmacy expenses that would typically come out of your pocket.
 What are some of the benefits of having an HRA?
– BlueLinx funds the account annually; unused funds roll over year-to-year to help
offset future medical costs ($10,500 maximum).
– BlueLinx provides 100 percent coverage for in-network preventive care services.
– You can use the funds to pay down your deductible.
 When you go to the doctor, do not pay at time of service. The doctor should file the
medical claim directly with UHC. UHC will automatically pay your bill from the funds in
your HRA account.
 If you need to pay at the pharmacy or have a bill, you will have a MasterCard (Health
Spending Card) loaded with your HRA funds
 Your funds are forfeited when you leave or cancel your medical coverage with
BlueLinx.

How Do I Access My HRA Account?
 UHC will automatically pay any bills on your behalf
out of your HRA account.
 If you need to pay a bill, use the MasterCard
provided to you by UHC (Health Spending Card).
– Card will draw from HRA fund first, followed by
FSA funds, if applicable.
– Card can be used for eligible expenses - when you go to the doctor, do not pay
at time of service. The doctor should file the medical claim directly with UHC.
UHC will automatically pay your bill from the funds in your HRA account.
– You should use your card to pay for any prescription drugs.
– You can submit a paper claim for reimbursement from your HRA account if you
don’t use your card.
 If you received a card previously, it is valid into future years – do not throw away your
card if all funds have been exhausted. You card will work until its expiration date.

 Mandatory generics - Generic drugs can cost about 20% to 80% less than brand-name drugs.
BXC will only pay for the generic cost if there is a generic available. If you still prefer to use a
brand drug even though a generic is available, you can pay the difference between the cost of
the brand and the generic (unless the use of a brand drug is medically necessary).
 Home delivery – After two refills at a retail pharmacy, all maintenance medications must be filled
via mail order. Members who refill at retail will be responsible for 100% of the cost of the
medication

 Dental coverage is offered through UMR. BlueLinx utilizes the Guardian Network of
providers.
 You can chose to see any dentist or orthodontist that you wish, however, you will
probably pay less if you see a guardian network dentist/orthodontist.

Flexible Spending Accounts (FSA)
 Two Account Options:
– Health Care Flexible Spending Account (HCFSA)
– Dependent Care Flexible Spending Account (DCFSA)
 Benefits of an FSA:
– Reduce your taxable income;
 Tax savings: Contributions to FSA are pre-tax and medical
reimbursements are tax-free.
 You will have more money in your pocket than you would if you paid those
same health care and dependent care expenses with after-tax dollars!
 How does this differ from my HRA account?
– FSA accounts are 100% funded by the employee on a pre-tax basis. You
determine the amount you want to contribute.
– FSA accounts have an annual Risk: “Use it or Lose it”. Funds must be used
for expenses incurred by 12/31 of the current plan year.

2015 Retirement Plan Offerings
401(k)
 As a BlueLinx salaried employee, you are immediately eligible to participate in
the BlueLinx 401(k) and Roth 401(k) Plans. You can begin contributing to the
Plans as soon as administratively feasible following your date of hire.
 BlueLinx provides a match to your contributions of 50% for the first 4%
invested (3 year vesting requirement). The match is funded on an annual
basis after the end of the plan year.
 You can contribute from 1% to 75% of your pay on a pre-tax basis to the
401(k) or on a post-tax basis to the Roth 401(k) plan up to the maximum
combined allowed of $17,500 for 2015 (subject to change per the IRS).
 Employees will be automatically enrolled in the plan approximately 45 days
after your hire date at a 4% contribution level. Contributions will be increased
by 1% annually up to 15% max.
 Employee may opt out of the auto-enrollment or auto-escalation at anytime by
contacting Vanguard.
 Enroll directly with Vanguard sooner than 45 days if desired.

2015 Supplemental Insurance Plans
What is a Supplemental Insurance Plan?
 Supplemental insurance is used to complement your health plans, not to replace
them.
 Plans offered are
 Accident Plan - covers injuries and accident-related expenses.
 Critical Illness Plan - covers diagnosis or treatment for a serious illness
 Group Universal Life with a Long Term Care Rider - lump sum to your
designated beneficiaries in the event of your death
 Different from the BlueLinx Life Insurance in that the Voluntary plan is your
individual coverage and is portable at the current rates.
 If you need Long Term Care coverage this benefit pays a monthly advance
of the death benefit of 4% for use for in home care, an assisted living facility,
adult day care, or a nursing home
 Plans are available to all benefit eligible BlueLinx employees and their dependents
regardless of their enrollment in a BlueLinx medical plan.

Dependent Audit
 BlueLinx requires all employees to verify their dependents once they are
enrolled in a BlueLinx medical, dental or vision plan
 Employees will be contacted by the Dependent Verification Center if they need
to complete the audit.
 Typical information required to complete the audit includes marriage certificate,
birth certificate, tax filings, etc.
 Questions can be referred to the Dependent Verification Center at 866-2727174.

 Advocacy will be available to answer basic benefit and eligibility questions.
“What is my PCP copay?”, “What is my deductible?”, or “Is my physician
contracted with my health plan?”
 Advocacy will research, resolve and/or educate regarding complex medical
claims, barriers with access to care, or extensive benefit clarification.
Facilitating claim reprocessing for a hospital or doctor’s bill.
Facilitating access to care to a prescription drug eligible under the terms of
the Plan.
Assistance with coordination of benefit information, especially as it relates
to Medicare.
 Allows participants to discuss their own (or their family members) medical
conditions and questions with licensed medical professionals.

 There is no cost for using this service, it is paid for by BlueLinx
 Advocates can be reached 24/7 via 866-279-0495
 Advocates are available 7:00 am – 6:00 pm CST Monday thru Friday
 After hours and weekend service is available to open a new case or
provide updates to an existing case
 Service is available for BlueLinx employees, their dependents, parents and
in-laws

Enrollment Steps
 You must enroll within 30 days of your hire date. Benefits are effective the first of the
month following your hire date.
 Review the enrollment guide that is mailed to your home address
