I-20 Application Informational Packet Students beginning their studies in the English Language Academy I-20 Application Information This is the first step in obtaining an F-1 visa, so that you may enter the United States to study for your degree. Please read the following information very carefully to ensure a smooth application process. Form I-20 The I-20 Form is the initial record that we have created for you in the United States Student Exchange Visitor Information System (SEVIS). You, the student, are responsible for completing the electronic I-20 application that will be used to create your I-20 Form. The electronic I-20 application does not save; you must complete it and submit it all at once. There is a percentage bar at the top of the application that indicates how far you are in completing the application. Please read through this information packet before starting your I-20 application. You can access the electronic I-20 application at www.mcphs.edu/I-20application Required documents for all students: □ Form I-20 application □ Copy of Passport Identification page(s) □ Proof of Funds: see “Funding your Education” section in this document for more details Transfer of School from another school If you attended another U.S. school last term in F-1 status, and are just home on holiday between academic terms, you are considered a “School Transfer” and need to have your SEVIS record transferred to us before your Form I-20 may be issued. Please contact your international student advisor at your previous school immediately to authorize the release of your records to us. You will need to provide your international student advisor at your previous school with the completed Transfer of Schools form for signature. This form is available on page 3 or located at www.mcphs.edu/transferofschoolform Please note, as a student transferring his/her I-20 from another school, you must provide us with the additional documents below. Required documents for students transferring from another school: □ Copies of all your I-20’s (page 1 and 3 of each I-20) □ Copies of your F-1 Visa stamp in your passport □ Copies of your I-94 admission record □ Change of Status Approval Evidence/ Document (if applicable) □ The rest of the I-20 application, including MCPHS University “Transfer of Schools Form” Biographical Information In this section, we ask that you provide us with your name and address exactly as it appears in your passport. A copy of your passport’s identification page is required to be sent to us. Funding your Education As an F-1 student, you are required to prove your ability to fully fund your Total Estimated Annual Budget for your studies in the U.S. This includes tuition/fees as well as average living costs. Please refer to the MCPHS University estimated budgets for 2014 – 2015 tuition and school fees, on page 6 or at www.mcphs.edu/2014-2015budget to review the cost of your attendance for your program. Please refer to page 4 of this document or visit www.mcphs.edu/affidavitofsupport for more information on how to prove your financial documentation. Unless you have enough funding to support yourself for your entire program of study, you will need a sponsor(s). 2) Funds from MCPHS University such as, a scholarship, a grant, an assistantship, or a fellowship. Not all College support covers the entire program of study or all expenses, so you may need additional support to meet your minimum annual costs. 3) Sponsors (parent(s), relative(s), spouse, organization(s), government, etc.) Each of your sponsors is required to complete and sign the reverse side of this “AFFIDAVIT OF SUPPORT.” Please feel free to copy this form if more than one sponsor is needed. Sponsors may also provide you with support in the form of room and board. Free room and board is when you live with someone and don’t have to pay for your room and/or food. A sponsor should promise only as much funding as he or she is able to give you. Evidence required All documents must be provided in English. All funding must be in US dollars. Documents should be photocopies (not originals). You will need the originals of ALL the documents you have sent us to take to the American Embassy when we send you your I-20. All documents must be less than six months old. Proof of Income Individual sponsors (including those that will provide free room and board) may supply income tax returns or receipts with their most recent pay stub, or their pay stubs for the last six months, or their bank/investment statements with a six month history. The sponsor may supply a letter from the bank or loan office. If a bank statement is in the name of more than one person, each individual must submit an affidavit of support. We can’t accept bank statements that do not specify monthly balances. The income of a company is not the income of the owner of the business and will not be accepted as proof of income. The owner/sponsor must still provide proof of income as well as the business registration and licenses. Conditional requirements Sponsors of free room and board must also provide a copy of their lease, deed, rent receipts, or a utility bill. If you have a full financial guarantee from a government or organizational sponsorship, please submit this letter instead of the affidavit of support form. If you are providing personal funds, you will need to submit a copy of your bank/asset statement (in your name) which lists balances and deposits. If you are listing MCPHS University funds as a means of your support, you will need to provide a copy of your award letter or contract stating exactly what you will be receiving and for how long. You must prove that you have sufficient funds for: A) Total Tuition and Fees B) Total Living Expenses C) Total Dependent Costs D) Other Costs The sum of these will be your total estimated annual budget. A + B + C + D = Total Estimated Annual Budget In order to calculate your total estimated annual budget, please use the estimated budgets on the following pages. Your real costs may vary from these budgets, but the amounts in these budgets must be used for your I-20. The duration of your stay will be estimated to two terms minimum. In order to calculate which amounts to use for your budget, you must add the amount of your start term and the amount of the next full semester when calculating your Tuition and Fees, your Living Expenses and your Other Costs.
