 The new operations committee will be formed from the original committee and the
Peel Human Services and Justice Coordinating Committee
 We ask David to continue, and that John Roloson also represent PCYPG
Ministry
Updates:
System Transition Coordination for Mental Health and Addictions: Kim Paumier
 Building on the Service Resolution model, this new program will develop a network
at a systems level to support transitions into the adult complex mental health and
addictions systems. The goal is to develop systemic solutions, rather than casespecific solutions.
 Elements of the program include:
o Service resolution coordinator (Kim, for youth)
o Systems resolution table, including Jeremy Daniels and Jen Speers from
Mississauga/Halton LHIN, Courtney and Kim from Central West, and John
Roloson and Memona Hossain, from Peel Service Resolution
o Defining gaps and identifying training in MH&A
o Create a barrier-free system of transition
o Data collection/analysis
o System transition guidelines
Susan Nickelchok:
Complex, Medically Fragile Children
 Agencies involved include: Erionoak, CAS, CCAC, Service Resolution
 Addressing systems issues about how these children are supported
 Working with hospitals re discharge, clarify mandates of each of the providers
 Goal is to collaboratively produce a flow map, specific to the needs of medically
complex kids, for use by front lines workers, which clarifies who to contact, who
does what.
 Orientation and cross- sectoral training would support the project
 Susan will convene a group to work on it, perhaps with support from a consultant
 Concern is to reach beyond the health sector.
Integrated Transition Protocol for DS (in CWR)
 Transition Advisory Committee included the DSO, school boards, and DS agencies
 Protocol outlines how to work with the ministry of education and school boards:
o What are the various roles, how to work together
o Produce one plan for a youth, which is “owned” by the school and the DS
sector
Sandra Frampton:
Youth In Transition Workers: to provide facilitation to youth leaving CAS care,
moving into adulthood. The Y and ACLC host the 2 positions.
Aboriginal supports: Peel/Halton is working with Enaahtig, an agency in Simcoe, to
address our requirements.
Alternate Dispute Resolution (ADR): AYSP is hosting this program, which is used in

child protection cases.
Moving on Mental Health:
 Changes have been made to detail codes and definitions
 Peel (and Wellington, Waterloo, York Regions in Central West) is in phase 1 of
identifying the lead agency. A decision will be announced in June.
 Status of the regional office is “Steady State” The new children’s’ team is meeting
this week to identify opportunities to work together on issues.
 There will be an Open House on March 4 to meet Wendy Nelson, Regional
Director
Agency
Updates
Kerry’s Place: Gail Jones
 The Peel ASD Working Group is working with Wendy Perry, in regard to
differences in diagnosis between parts of Peel.
Discussion:
 PCC has an internal community of practice- the ASD Working Group is a hub for
autism in Peel
 Patty Bingham is leading a ministry program to develop a framework for the
disagnosis of autism.
 Important to identify best practices and urge adoption of the best tools
 Involve CPRI in the discussion
 Conflicting diagnoses are an issue for this population
 Sometimes it is more important to focus on the treatment recommendations even if
there are different diagnoses (though the diagnosis may be a barrier to accessing
the treatment)
Halton Trauma Centre: Darryll Hall
 Agency has moved offices and will be in its permanent space by March 1
 To prepare for its connection with Centralized Intake it will work with PCC to
distinguish eligibility criteria and the pathways for various referrals.
 Centralized Intake is working with Helen Mullen-Stark to revision itself. The broader
CI committee will meet in the new fiscal year and will invite Halton Trauma Centre
Peel Children’s Aid: Mahesh Prajapat
 The Mental Health Strategy is moving forward, with support from Carolyne Hooper,
and input from AYSP and PCC
 Currently lots of media attention on CAS issues, with the current inquest wrapping
up, murders in Nova Scotia, and the children of a religious sect being apprehended
in Chatham-Kent. Mahesh is happy to answer any questions about these issues
 Peel CAS has an excellent record of a very low number of children in Care
Peel Crisis Capacity Network: Nicole reported
 PCCN has partnered with UTM, with support from the two hospitals, on a study. It
will build on a Toronto model to compile Peel information for individuals with

DS/DD when they are at Emergency Depts.
 Plan to produce tools for families in effective communication and sharing
appropriate information when at an emergency department.
 Nicole is leaving the Coordinator’s position for another opportunity. She has
served in this position for seven years
Erinoak Kids: Adrienne reported
 EOK gets many referrals, usually from SLPs or pediatricians, of young children
needing screening for potential autism.
 A screening tool is being developed to help identify those kids who should be
referred to a developmental pediatrician. This will shorten the wait list, and enable
early interventions to be provided.
