TALKTOOLS® JAW EXERCISERS Disinfect therapy tools before use. This set of tools (two Jaw Exercisers) is designed to teach jaw stability and grading through movement and resistance. Suggestions: 1. You will need one set of TALKTOOLS® Jaw Exercisers to implement this exercise. 2. Jaw exercises should be used even when there is excessive tongue protrusion. The jaw exercises will address tongue retraction goals. 3. The following exercises should be addressed prior to introduction of this technique: 4. 5. 6. 7. 8. A. TALKTOOLS® Jaw Grading Bite Blocks, which uses the six Graduated Bite Blocks. B. TALKTOOLS® Progressive Jaw Closure Tubes, which uses the six Graduated Bite Tubes. This exercise sequence is used with clients who are unable to achieve volitional jaw closure upon request (i.e., clients with the diagnosis of Cerebral Palsy, Traumatic Brain Injury.) Poor jaw grading is secondary to jaw muscle weakness. The resulting jaw instability translates into an inability to establish the necessary jaw height for standard speech sound production of both vowels and consonants in co-articulation. Frequently the client develops a compensatory “High Jaw Fixed” posture or a compensatory “Low Jaw Fixed” posture for function, which inhibits mobility in the jaw. This technique will address jaw strength, stability and grading goals through movement. In order to avoid injury to the jaw the following principles must be acknowledged in each Step of this exercise: A. Monitor to insure that the client is not using whole body compensatory postures to bite on the Jaw Exercisers. B. If the client uses excessive force in the jaw, the tempro-mandibular joint may be injured. C. The client should only be biting hard enough to keep the Jaw Exercisers securely between the teeth. D. Do not work beyond the prescribed number of repetitions. Teaching the client to chew on his/her back molars will be a valuable supplement to this exercise. If the jaw slides out of symmetrical alignment, during any part of this exercise, STOP and return to the exercises entitled, “Jaw Grading Bite Blocks.” Homework: Establish during the session, where the client fails on the hierarchy. Practice that “Step” at the highest level of successful repetitions for at least one week before moving onto the next level or “Step.” #1 Jaw Exerciser Step #1 1. Place the #1 Adapter on the #1 Jaw Exerciser. Push the adapter towards the attached end until it locks just past the first notch, as pictured in “A” below. “A” “B” 2. Place the open ends of the #1 Jaw Exerciser at midline between the client’s upper and lower central incisors extending from the front of the mouth. Instruct the client to bite down gradually for 1 second on the exerciser as pictured in “B” above, until the ends meet. Hold that closure posture for 1 second then gradually release the strength of the bite for 1 second, to allow the Jaw Exerciser to return to its open position. When the Jaw Exerciser is in its open position, remove it from the client’s open mouth. (1 unit), 1 time. Note: If the jaw slides out of symmetrical alignment during any part of this exercise, STOP and return to the exercises entitled, “TALKTOOLS® Jaw Grading Bite Blocks.” 3. Replace the #1 Jaw Exerciser at midline between the client’s upper and lower central incisors. Instruct the client to bite down gradually for 2 seconds on the exerciser, until the ends meet, and to hold that closed posture for 2 seconds. Then gradually release the strength of the bite for 3 seconds to allow the Jaw Exerciser to return to its open position. When the Jaw Exerciser is in its open position, remove it from the client’s open mouth. (1 unit), 1 time. 2. Criteria for Success: Repeat the technique described in this “Step” 10 times in succession without a break. Remember to remove the #1 Jaw Exerciser after each unit. Step #4 1. Replace the #1 Jaw Exerciser at midline between the client’s upper and lower central incisors. Instruct the client to bite down gradually for 4 seconds on the exerciser, until the ends meet, and to hold that closed posture for 4 seconds. Then gradually release the strength of the bite for 4 seconds to allow the Jaw Exerciser to return to its open position. When the Jaw Exerciser is in its open position, remove it from the client’s open mouth. (1 unit), 1 time. 2. Criteria for Success: Repeat the technique described in this “Step” 10 times in succession without a break. Remembering to remove the #1 Jaw Exerciser after each unit. Step #5 1. Replace the #1 Jaw Exerciser at midline between the client’s upper and lower central incisors. Instruct the client to bite down gradually for 5 seconds on the exerciser, until the ends meet, and to hold that closed posture for 5 seconds. Then gradually release the strength of the bite for 5 seconds to allow the Jaw Exerciser to return to its open position. When the Jaw Exerciser is in its open position, remove it from the client’s open mouth. (1 unit), 1 time. 2. Criteria for Success: Repeat the technique described in this “Step,” which will require the client to hold the #1 Jaw Exerciser in the closed position for 5 seconds, 10 times in succession without a break. Remember to remove the #1 Jaw Exerciser after each unit. Step #6 1. Reposition the #1 Adapter on the #1 Exerciser until it locks just past the second notch at midpoint as pictured below: Note: By moving the #1 Adapter to its new position, the exercise will become more difficult. 2. Criteria for Success: Repeat Steps #1 through #5 as described above using this new adapter position. Step #7 1. Reposition the #1 Adapter on the #1 Exerciser at 1” from the tip until it locks just past the third notch as pictured below: Note: By moving the #1 Adapter to its new position, the exercise will become more difficult. 2. Criteria for Success: Repeat Steps #1 through #5 as described above using this new adapter position. #2 Jaw Exerciser - After you have completed the technique described above using the #1 Jaw Exerciser, you may introduce the #2 Jaw Exerciser.
