This process aligns with the LHSC Corporate Policy PCC081 Patient Leave of Absence.
Ordering the LOA
1. Provider to add

2. Add a checkmark to Leave of Absence.
3. Right-click on Leave of Absence.
4. Choose Modify Planned Order.
5. Select the Order Sentence and click the
button.

6. Complete Details for Leave of Absence order including:
a. Start date and time,
b. Length of time,
c. Accompaniment etc. in special instructions.
7. Add a checkmark to Leave of Absence-Pharmacy Medication Request.
8. Right-click on Leave of Absence-Pharmacy Medication Request.
9. Choose Modify Planned Order.
10. Select the Order Sentence.
11. Complete Details for Leave of Absence-Pharmacy Medication Request:
a. Priority,
b. Start date,
c. A time.
12. Add any instructions to pharmacy e.g. 72 hours.
13. Click the
button.

Preparing the prescription for the patient’s medications
1. In patient’s chart, go to Task and select Reports.
2. Select LOA Prescription, complete From: and To: date and time.
3. Select Printer destination.
4. Click the
button.
5. Provider reviews the medications, orders the doses and signs prescription. Send signed
prescription to pharmacy to be prepared and returned to unit once completed.

Completing medication tasks on the MAR while patient is on LOA
1. Right-click on the medication task and choose Chart Not Done from the short-cut
menu.
2. From the (Not Done) dialog box, click the down-arrow beside the Reason Not Done
field and choose Patient Out on Pass.
OR
3. From the (Not Done) dialog box, click the down-arrow beside the Reason Not Done
field and choose Other.
4. In the Other dialog box, type LOA and click the
button.
