After collection of the Application Form, filled up Form with all self-attested documents & Course Fee
(Cash) must be submitted to the above mentioned branches of Canara Bank & collect the respective
part of the challan.
ADMISSION TO STUDY CENTRES
For Admission to all Study Centres (Code 01 to 41) Prospectus and Application Forms will be available
on payment of Rs.120/- from 20.08.2014 from the respective Study Centre. After collecting Application
Form candidates will deposit their course fee through Demand Draft in favour of D.D.E, R.B.U., payable
at Kolkata and submit the filled up Application Form along with DD, Challan and attested copy of Mark
sheets, Certificates etc. of previous examinations to the same Study Centres which will check the
eligibility of the candidates on the basis of original documents. Candidates must collect the applicant
part of the challan from the study centres after the eligibility is confirmed.
For filling up Application Form the Prospectus and Guideline should be carefully followed. For details

late Fee) and 29.09.2014 (with late fee of Rs. 100/-).
