Kaiser, etc.) when you view your benefit options on BenefitsWeb. To check whether your doctor, dentist or vision provider is in your insurance
carrier’s network, you should call the insurance carrier or check their website. When you contact your insurance carrier, be sure to provide the plan
or network name shown in bold under “What You’ll Find” in the table below. For a complete insurance carrier contact list, refer to the 2014
