授業料免除の申請について
平成２８年度第２期分授業料免除を下記のとおり取り扱いますので、免除
等希望者は期限までに申請してください。
また、申請書類等は本学ホームページ上からのダウンロードのみとなり冊
子での配付はありませんのでご注意ください。
○ 申請書類等ダウンロード開始期：平成２８年６月下旬頃に本学ホームページ掲載
○ 申請書書類受付開始日：平成２８年７月１日（金）
＊書類不備等で受理できない例が毎年見受けられますので、
受付期間
平成２８年７月２２日（金）までに対応支援室学生支援・教務
にて必ず申請書類の確認を受けてください。
【この期間に不足書類等を取り寄せてください。
不備がある場合は受理できません。】

申請書類に不備がある場合は受理できませんので、早めに申請手続をしてください。

平成 28 年度第 2 期分の申請書は、平成 28 年度第 1 期申請書と様式が異なります。
また、添付書類を省略した「継続申請」はありませんので、注意してください。

平成 28 年度第 1 期分授業料免除申請をした場合でも、平成 28 年度第 2 期分の免除等を
希望する場合は、上記による申請が必要です。

Application for Tuition fees Exemption
Prospective students for tuition fees exemption for the Second period of AY 2016 are required to apply by the
deadline and through the following procedure.
The way of obtaining application forms is not by distribution, and only by downloading from the University
homepage.
Please pay attention.

Start accepting : from July 1 (Fri) ,2016
＊ Any applications without required documents are not

1.
Incomplete documents will not be accepted, so please prepare your application as early as possible.
2.
The application form for the Second period of AY 2016 is different from that for the First period of AY
2016. Please note that “continuance application” is not conducted this time.
3.
Even if applicants have already applied for tuition fees exemption for the first period of AY2016, those who request
for exemption for the Second period must apply again by above procedures.
