2014-2015 JET PROGRAMME JAPANESE LANGUAGE COURSE GUIDE TERMS OF CONSENT RELATED TO PERSONAL INFORMATION （TRANSLATION ＆ INTERPRETATION） Please send the Course Application Form after confirming and consenting to the following points. １．Terms of Consent Related to Japanese Language Course (1) Intended Use Administration of the JET Programme Japanese Language Courses. Information provided on the application form will be used for the following purposes: 1. Providing necessary course-related information (your name, office address and postal code, office phone and fax numbers, course grade, and a history of courses taken) to the companies and organisations managing the Japanese language courses 2. Mailing of course-related materials 3. Management of the language courses 4. Responses to Help Line enquiries 5. Collecting course-related surveys and feedback 6. Making course-related requests or announcements (2) Information Provision Information provided on the application form will be provided to the following organisations. * No JET Programme participant’s information will be given to any third party not related to the JET Programme. 1. Companies which are entrusted with the management of these language courses 2. Host prefectures/designated cities 3. Contracting organisations (3) Notes If you choose not to provide your information on the application form, you will not be able to participate in the JET Programme Japanese Language Courses. ２．Other (1) Regarding nationality and gender: if these details are not filled out on the form, CLAIR will take the liberty to cross reference and record these details from the participant’s original JET Programme application. (2) Some details collected on this form, i.e. home address and post code, and home phone number, will not be included in the JET Programme Participant List. (3) However, in the case of accidents, natural disasters, missing persons or other life threatening emergencies, please note that we may offer your personal details to the organisations listed in “2.
